Job Definition A. Scope of Position:
​The clinical coordinator assesses and monitors clients' health status and physical care needs at admission and while clients are in the program.  Supervises clients in all aspects of medication self-administration and all management of medication and other client care supplies.  Provides health education for clients, families and community groups as needed or requested. Functions as a member of the Interdisciplinary Treatment Team.  Performs administrative, clerical and other tasks according to program policy and needs.   Drives vehicles to and from assigned destinations to pick up and deliver patients and/or supplies.  May be required to assist patients into and out of vehicles. Performs quality clerical services for the program as required.  The patient population served by this position is adult and geriatric

Performs other duties as assigned or requested. B. Reporting Relationship:
​See Table of Organization. C. Education Experience:
​R.N. license issued by the appropriate state is required. Valid driver's license as required by State law. Must meet facility hiring guidelines.  Certification in gerontological or psychiatric nursing is desirable.  Knowledge of standard nursing practices and functions.  Familiarity with medical terminology and conditions relating to geriatric care.  Familiarity with medical team roles and with the functioning of other hospital departments.  Knowledge of effective individual and group communication techniques. Knowledge of safe operations of vehicle and determined best route for picking up/delivering patients efficiently and safely.  Knowledge of how to physically assist all passengers on and off the vehicle.  Knowledge of performing various secretarial and clerical tasks. D. Physical Requirements:
Maintains a high level of physical and emotional health to cope with periods of high stress and working hours that may extend beyond the standard work week.  Requires close mental, visual and auditory attention in all phases of patient care activities.  Visual and hearing impairments should be corrected within normal range.  The team member frequently is required to stand, walk or sit for long periods of time.  Physical demands may require knowledge of use of appropriate equipment.  Ability to concentrate with frequent interruptions.  Remain composed, tactful, discreet and cooperative at all times.  Must be able to communicate effectively with patient, families, team members, physicians, other healthcare provider staff and visitors displaying a smile and maintaining a pleasant attitude. E. Working Conditions:
​On a regular basis the team member will work in a temperature controlled direct patient care area.  Stress levels may be high at time due to situations involving patients, along with interactions with physicians, families, and other health care providers, and the competitive healthcare environment.  Demands high level of concentration and the ability to work in emotionally charged situations.  The skills of collaboration, negotiation, conflict resolution and effective communications are critical.  Some travel required. ]]>
