The Clinical Nurse Care Manager is responsible and accountable for coordinating the care of a specified group of patients. Assesses, plans, implements, coordinates, monitors, and evaluates options to facilitate the continuum of care. Educates staff regarding effective allocation of resources while encouraging provision of high quality patient care.Job Duties
In coordination with the Hospitalist on Call, assign and distribute patients geographically to the Hospitalists through EMR. Partners with the Chief Hospitalist to improve HCAHPS and quality and throughput initiatives. Works with clinical documentation specialists to help Hospitalists complete clinical documentation queries in a timely manner. Maintains a professional and cooperative relationship among health care teams by communicating information, responding to requests, and building rapport by participating in team problem-solving methods. Performs responsibilities in a manner that promotes quality and safe patient care and excellent customer service. Demonstrates knowledge and exemplifies the Baylor core values, respects patients, service providers, organizational confidentiality and HIPAA Security procedures and protocols. Participates in division meetings, quality improvement projects and other activities of the Hospital Medicine Program.Minimum Qualifications Education:
- Required: Bachelor's degree in Nursing.
- Required: Four years of relevant experience.
- Required: Current license as a Registered Nurse (RN) by the State of Texas Board of Nursing.
- Preferred: Eligibility or Certification in Case Management by the Texas Department of State Health Services (DSHS).
