This is a per diem position to administratively support the Bridge Program grant. Employment with Baystate for this position will be for the duration of the grant. Ideal candidate will have Monday-Friday availability, on average 10-12 hours per week.
Bridge Program Coordinator provides key administrative support to the Bridge Program Director and Bridge Team members by acting as a liaison between patients, hospital and community partners. The ability to exercise judgement, critical thinking, and independent decision-making skills are essential. Responsibilities require a thorough knowledge of the Baystate institution and EMR.
Demonstrated experience working with adults and families, including experience navigating social resources, behavioral health, and medical supports both inpatient and outpatient. Good listening skills with a focus on self-efficacy and person-centered care.
Must be able to work independently with follow through on projects as well as be an effective and valued member of a cross-continuum team. Excellent oral and written communication skills and computer competency.
1)Receives calls and correspondences and conveys information to the appropriate team members.
2)Maintains relationships with contacts inside and outside the hospital to relay information, schedule meetings, gather data, maintain census. Copies and circulates information, agenda, minutes. Seen as the point-person for the Bridge Team.
3)Helps prepare for, schedule, attends, keeps attendance records and participates in Bridge Team program and team meetings.
4)Initiates and prepares expenditures for office, standard inventory, travel, etc. and accepts receipts and invoices for routine expenditures and reviews for accuracy. Enters data into centralized Baystate system.
5)Supports and provides in-servicing, patient education and referral.
6)Capture activities of the Bridge Team overtime and in such a way as to help tell the story of this program to any interested party.
