Job Summary and Qualifications

The PHP/IOP Licensed Clinical Social Worker demonstrates responsibility and accountability in the performance of assessment, reassessment, development and implementation of the patient's plan of care based on patient/family/significant other social needs and in accordance with organizational/ departmental standards with special focus on the discharge planning and continuum of care needs. As a staff member, the PHP/IOP Licensed Clinical Social Worker can expect to care for adult and geriatric psychiatric patients. The PHP/IOP Licensed Clinical Social Worker interacts with other members of the healthcare team and demonstrates knowledge of the specific psychosocial conditions and treatment therapies for the patient populations. They provide psycho-social services to patients and their families as appropriate.

  • The PHP/IOP Licensed Clinical Social Worker consistently responds to orders within 72 hours of referral. Completes a psychosocial assessment that includes the social, emotional, cultural and discharge planning needs of the patient and family. Patient information is recorded in the patient's medical record as per hospital/departmental policies. Communicates with the healthcare team to obtain additional information to meet the patient's short- and long-term needs.
  • The PHP/IOP Licensed Clinical Social Worker maintains knowledge of pertinent legal requirements, regulatory compliance standards and funding for community resources. Educates patient/ family/ significant, physician and staff on the regulations and available community resources. Effectively communicates changes to the treatment team to maintain ongoing continuum of care.
  • The PHP/IOP Licensed Clinical Social Worker consistently establishes a discharge plan that is ready for implementation at the time of discharge by reassessing and monitoring the development of the discharge plan to ensure effectiveness of implementation. Revises the plan as needed in a timely manner. Supports and promotes the patient/family/ significant other involvement in the treatment and discharge plan decision making process. Consistently provides accurate information and maintains communication with community patient care providers and funding or third party payors to ensure continuity of care.
  • The PHP/IOP Licensed Clinical Social Worker assists with the education of designated health care surrogate/proxy/guardian.
  • The PHP/IOP Licensed Clinical Social Worker participates in the interdisciplinary treatment team meetings. Effectively communicates discharge plans and/or barriers to discharge with other members of the health care team.
  • The PHP/IOP Licensed Clinical Social Worker complies with departmental policies and procedures related to required legal documentation and reporting, performance improvement initiatives, and other activities as established throughout the year.
  • The PHP/IOP Licensed Clinical Social Worker facilitates Psychotherapy group sessions. Demonstrates knowledge of group process and is consistent in facilitating psychotherapy groups.
  • The PHP/IOP Licensed Clinical Social Worker provides individual psychotherapy as appropriate. Manages difficult emotional situations. Channels concerns appropriately and confidentially. Effectively communicates findings with the treatment team.
  • The PHP/IOP Licensed Clinical Social Worker participates in family conferences as appropriate. Coordinates meeting time and location.
  • The PHP/IOP Licensed Clinical Social Worker consistently demonstrates knowledge and understanding of human behavior, interacts appropriately and creates emotionally supportive relationship with the patient/family/significant other.
  • The PHP/IOP Licensed Clinical Social Worker participates in the Utilization Review process.
  • The PHP/IOP Licensed Clinical Social Worker participates in community outreach and is involved in sales-related activities to promote the program in the community.
What qualifications you will need:
  • Masters in Social Work (MSW) required
  • Licensed LCSW in the State of Florida Required
  • At least 3-5 experience in Behavioral Health/Psychiatric Experience Preferred
  • Hospital experience or health care setting. Acute care social work experience preferred)
  • Bilingual (preferred) Spanish/English
