*Highland Hospital is seeking a Full Time, Social Worker (MSW) to join its team.  Candidates for this role will work Monday - Friday, Days.  Some duties include but are not limited to:The Social Worker provides clinical social work services in various components of the Hospital Services, according to identified service need.  He/She is responsible for ensuring that the needs of the patients are met and maintained on an individual basis in accordance with policies and procedures.1. Risk Screening and psychosocial assessment via established criteria                                                      2. Collaborates in the plan of care and discharge planning process with the following functions:  

  1. Participates in daily multi-disciplinary health team rounds
  2. Facilitates identified discharge needs for home discharge patients
  3. Facilitates placement process for nursing home and other facility discharges including:
    1. meeting with patient/family to review process and identify long or short term placement options
    2. facilitating the development of a facility referral packet for potential placement facilities
    3. negotiating bed offers between facilities and the patient/family
    4. facilitating any financial concerns related to placement
    5. facilitates appropriate transfer of patient to facility
  1. facilitates transition to ALC Unit as appropriate pending hospital d/c
  2. facilitates acquisition of medications through charitable funding for uninsured or under-insured patients to facilitate discharge
  3. identifies and addresses any barriers to discharge in collaboration with care team
3. Provides information and referral to patients and families regarding community resources and mobilization of those resources as needed.4. Provides crisis intervention as needed5. Provides death and bereavement counseling and assists families with funeral and burial arrangements  6. Identifies and responds to elder, child and domestic violence indicators     7. Facilitates hospice planning and placement including patient and family education 8. Addresses community living needs including housing, food, transportation and home care     9. Provides social system intervention, especially family systems issues as they effect patient care   and discharge planning.10.Facilitates the identification of and referral intervention for mental health and addiction issues       11. Manages legal issues that affect patient care and/or discharge including health care proxy, advanced directives, POA and guardianships.            12. Documents patient progress and ongoing plan of care / discharge developments 13.  Provides On-Call services on a planned schedule.                 


  1. Education: MSW required, or have LMSW  
  2. Experience (minimum required): One year in a healthcare setting preferred.
  3. License/Certification Required: LMSW required within 18 months of hire.
  4. Skills (to include equipment, software, etc.)  Basic computer skills including Microsoft Office products.  Excellent communication skills.
