  • Renders or assists with personal care/ADLS such as: bathing, change of bed linen, shampooing, shaving, mouth care, etc.
  • Assists with ambulation, transfers, and/or normal range of motion and positioning.
  • Obtains patient vital signs. Records on appropriate form if assigned to do so.
  • Reports any changes in patient/client condition and needs to the Registered Nurse or     appropriate supervisor.
  • Maintains a clean, safe, healthy environment.
  • Observes for adequate nutrition/fluid intake.
  • Observes basic infection control techniques.
  • Perform other duties as assigned.
  • Committed to the continuous improvement of the patient experience which includes patient safety, quality of care and excellent service.  Adhere to the organization's published values.
  • Recognizes emergencies and acknowledges emergency procedures.
  • Completes progress notes on all patient/client visits on care furnished.

Knowledge, Skills and Abilities Required

License/Registration/Certification Required: 

License/Registration/Certification Preferred:

Minimum Education Required:  High School Diploma or equivalent.

Advanced Education Required:  (Associate/Bachelor/Master/PhD level and applicable field of study or related field) or equivalent combination of training and/or experience: 

Advanced Education Preferred:  (Associate/Bachelor/Master/PhD)

Minimum Experience Required:  

Additional Experience Preferred:  Previous experience in long term care nursing home, hospital, hospice or home care agency.

Skills (Interpersonal and job complexity):  Must possess good communication skills. Aides must be able to work in the home independently and care for the patients/clients according to the policy. Must demonstrate tact and good judgment and empathy relating to those who are ill. Must possess the ability to deal effectively with the demands of the job.

Basic computer, data entry, typing, filing and organization skills required.

Abilities:  

Ability to work extended hours beyond normal work schedule to include, but not limited to, evenings, weekends, extended shifts and/or extra shifts, various locations, sometimes on short notice.

Environmental Conditions:  Potential exposure to fumes, dust, gases, and odors.   Frequent changes in temperature may occur.  Possibility of exposure to communicable diseases.
