
Description
The Case Manager has clinical expertise with knowledge of medically appropriate criteria, and applies principles of utilization and quality management, discharge planning and the management of clinical/financial resources as a facilitator and active participant of the multidisciplinary patient care team. The Case Manager predicts, facilitates and evaluates the transition of patients across the care continuum and links people to systems that provide them with resources, services and opportunities to achieve and maintain maximum functioning.

Preferred and/or Registered Nurse License Required Certifications/Registration Requirements Certificates/Registrations Required/
Preferred and/or ACM (Accredited Case Manager) Preferred or NACCM (National Academy of Certified Care Managers) Preferred or CCM (Certified Case Manager) Preferred Additional Requirements
Require Case Management Certification// including CCM// ACM or NACCM within 3 yrs. employment (time exception provided for unsuccessful attempts)
Other RequirementsCommunication and interpersonal skills, basic computer, analytical, grammar and spelling skills. Ability to effectively multi-task and prioritize responsibilities.
