We have an immediate opportunity for Home Health Clinical Educator

Reports to: Corporate Director of Clinical Services and Quality Management

Job Summary

The Clinical Educator:

  • Is responsible for the implementation of Agency staff education including orientation, competency, and staff development.
  • Assists the Corporate Director of Clinical Services and Quality Management in planning, developing, and providing education programs to address policy changes, new procedures or technologies, regulatory requirements, and accreditation standards.
  • Provides ongoing evaluation of the quality and effectiveness of the educational activities to ensure that they maintain and enhance professional development that promotes the delivery of safe, high quality patient care.
  • Recommends programs or other methods of education to meet identified needs and promote staff safety, patient safety, and high quality care.
  • Ensures that educational activities are congruent with the agency's mission, priorities, and resources.

Job Description:

  • Acts as a trainer and resource to other nurses and/or other disciplines by providing direct guidance relating to clinical practice, education, assessment, evaluation, quality performance improvement and direct clinical care in the patient's home environment
  • Demonstrates advanced assessment skills
  • Serves as a resource for OASIS accuracy, ICD coding proficiency and their impact on reimbursement and quality measures. Prepares and disseminates educational programs/materials for continuous process improvement
  • Anticipates and understands the impact of nursing care on patient outcomes
  • Coaches and trains regarding changes to established protocols and processes
  • Monitors and assures appropriate patient scheduling protocols for assigned agencies
  • Monitors and facilitates completion of orientation/preceptor paperwork to provide up to date information on clinician's progress and development
  • Evaluates and completes clinical competency check lists for those in preceptorship training
  • Assists in completing 30, 60, and 90-day clinical evaluations during assessment period(s)
  • Utilize clinical orientation and preceptor materials/checklists in establishing clinical training and program expectations
  • Identifies individual educational learning and training sections for self-study as well as for use during scheduled clinical visits
  • Maintains effective communication with field and office staff
  • Ensures written communications are clear and concise
  • Complies with applicable legal requirements, standards, policies and procedures including, but not limited to the Compliance Program, Code of Conduct, HIPAA, and Documentation Standards
  • Maintain a commitment to the values and mission of LifeCare Home Care
  • Perform additional responsibilities as assigned by one's supervisor or another manager related to the position/department

Requirements

The Qualified Candidate:

  • Must possess a current unrestricted RN license in state of agency operation; baccalaureate degree preferred
  • A minimum of three years of experience as an RN
  • Minimum of two years of home health experience required
  • Strong clinical background in home health is required, preferably in a leadership role
  • Current CPR Certification required
  • One year of experience providing teaching/training to clinical staff preferred
  • Demonstrated ability to plan, organize, implement and evaluate clinician work in a home health care setting
  • Demonstrated ability to instruct in small groups and one to one
  • Demonstrated experience utilizing effective interpersonal communication, organizational and priority-setting skills
  • Strong clinical background in home health is required
  • Current OASIS and ICD-coding certification required on hire or within 6 months of hire
  • Excellent written and oral communication skills
  • Must be able to deal with a challenging work environment from time demands and conflicting priorities
  • This position requires a working knowledge of Medicare Conditions of Participation for home health care, state licensure regulations, home care accreditation standards, OIG Work Plan and HIPAA regulations
  • Ability to maintain a positive and professional image at all time
