\n \n \n \n \n General Summary of Position: \n\n
- The Senior Clinical Social Worker (CSW) provides patient/family interventions to facilitate quality patient care and timely discharge planning. Coordinates with physicians, nurses, Clinical Resource Management (CRM) Case Managers and other disciplines within the care team, including outside agencies, to expedite the appropriateness, effectiveness and timeliness of post acute care and discharge planning. Using a variety of treatment modalities, Senior CSWS assists patients and families in the resolution of social, financial and emotional problems related to illness, health care and rehabilitation. Assists to effectively and efficiently provide complete comprehensive discharge plans to decrease LOS by having services available in a timely manner. Provides additional care for the psychiatric, ETOH/drug abuse, rape, neglected adult and child, and self-pay patient; and the highest possible care for planning a safe discharge in a timely manner and assists with projections for discharge and IDRs daily. This position includes meeting the needs and providing services to all age groups-infancy through geriatrics, as well as providing services for the culturally diverse population MedStar Washington Hospital Center serves. \n
- Master's of Social Work degree from a program accredited by the Council on Social Work Education is required. \n
- Minimum of one year employment as a Clinical Social Worker at MedStar Washington Hospital Center. Proven ability to develop and implement creative and timely discharge plans using community and financial resources to meet specific patient needs. Demonstrated ability to relate with patients, families, co-workers, care team and community agencies in a professional and sensitive manner; proven ability to communicate in a clear, concise and logical manner orally and in writing. Strong computer skills, to include presentation software. Proven critical thinking skills; demonstrated ability to assess data reflective of the patient's status and correctly interpret relevant information to identify each patient's requirements relative to his or her specific needs and to provide the care needed. Proven ability to manage caseload, including prompt assessment and re-assessment of patient, development of timely and safe discharge plans, ability to balance patient needs with hospital length of stay goals, and demonstrated ability to use clinical expertise to be an effective and efficient partner to the other members of the care team. \n
- District of Columbia Licensed Independent Clinical Social Worker (LICSW) is required. \n
