Description

The Bereavement Coordinator is responsible for the overall management of a hospice bereavement program. Responsibilities include the planning, implementation, maintenance and provision of grief services necessary to patients, identified caregivers, hospice staff, and the community we serve. Bereavement services extend for the time-period of the patient#s admission to hospice services until at least 13 months after the death of the patient. # This position would have the ability to cross train to our Social Services department, supporting both in-patient and Long Term Care settings. Free supervision for licensure requirements would also be available.# # Responsibilities:# Manages and directs all grief support programs and bereavement activities Ensure that bereavement services reflect the needs of the bereaved. Make bereavement services available to the family and other individuals in the bereavement plan of care up to 1 year following the death of the patient. Creates annual grief group calendar, including identifying group facilitators Ensures that all program policies and procedures are adhered to Provides planning and oversight for all functions of the grief support programs Leads the grief support programs in agency quality assessment and improvement initiatives Provides planning and oversight for all functions of the grief support programs Provide phone support: information, intakes, crisis response, and community referrals Develop a bereavement plan of care that notes the kind of bereavement services to be offered and the frequency of service delivery Qualifications:# Master#s degree in Social Work Licensed by the MN State Board of Social Work Current Driver#s License and proof of vehicle insurance 2 years of social work experience in a health care environment preferred Schedule:# 32hrs/2wks Flexible#schedule of Tuesday and Thursday: 7:30am-4:00pm Opportunity for a hybrid schedule, being able to work from both the office and remotely Northfield Hospital # Clinics is an independent organization with over 800 employees.
