Description
Provides direct patient care and/or light housekeeping under the assignment and supervision of an RN to assist in meeting the needs of a patient in his/her home setting. #This position provides direct care for adult and geriatric patients. Responsibilities: Observes patient status # environment and documents # reports any changes to RN case manager. Assists clients with nutritious meals, snacks # fluids as assigned. May perform errands within city limits of Northfield. Maintain a clean, safe environment and provide light housekeeping. Documents patient care in assigned patient records. Qualifications: High School Diploma or equivalent; must be at least 18 years old Completion of a Nursing Assistant program Registered with the Minnesota Department of Health Nursing Assistant Registry in good standing Prefer Nursing Assistant experience Prefer two years experience in Homecare and Hospice BLS certified (or willingness to obtain within 6 mo. of start date, company paid) Valid Driver#s license, insured reliable vehicle, and proof of car insurance Ability to travel to patient#s homes within a 35 mile radius (Medford to Lakeville and surrounding communities) Schedule:# 48hrs/2wks Weekdays: 7:30am-4:00pm Every sixth weekend rotation Northfield Hospital # Clinics is an independent organization with over 800 employees.
