Engage staff in critical thinking and elevation of best practice applications to care management managing optimal visit utilization. Facilitate interdisciplinary care team meetings. Develop Chronic Care Management process across EHR. This person will be part of the RVNAH leadership team. Requirements: Associates degree or equivalent experience; 3-5 years related experience NH license - RN, PT, ST or OT CPR certified or within 30 days of hire Driver license - valid Major Responsibilities: Previous Management experience in home care. Drive clear and purposeful visit utilization according to established best practice guidelines and case management. Educate the clinicians on effective care management and authorize only necessary and goal oriented visits for the patients. Explore care options at mandatory weekly care team meetings. Use OASIS document as the basis for teaching of creating/adjusting individual Plan of Care to focus on quality outcomes and utilization. Review and approve care plans that require deviations beyond the range permitted. Engage critical thinking of clinicians and elevation of best practice applications. Review and approve discharges and recertifications. Develop Chronic Care Management program across the continuum of care (EHR). Facilitation of Interdisciplinary Team meetings. Assume responsibility for staff to actualize what is learned through care team meetings; managing to optimal patient care outcomes. A resource for staff with obtaining insurance authorization. Back up to Clinical Managers and Clinical Director as needed. Member of RVNAH leadership meetings; IOP and clinical manager meetings.
