Reporting to the Director Performance Improvement, responsible for Precepting employees, assessing needs, develop, implement and evaluate education programs, orientation and in-services for RVNA H employees. Provides professional care as assigned during approximately 50% of work time, coordinating care planning with other disciplines. Able to float to other positions as needed. Requirements: PT, OT or ST license NH 5-7 years experience BCLS within 30 days of hire Driver license - valid Major Responsibilities: Understands, follows and is a role model for agency policy, process, regulation and mission. Mentors, clinically precepts, and is a resource to staff. Provides McKesson clinical instruction. Provide performance feedback to management on a regular basis. Develops and sets practice standards and coordinates clinical competency training and development of patient education materials, utilizing evidence based research with Staff Development Specialist and agency leadership. Assists with coordination of orientation of new employees with Staff Development Specialist and agency leadership. Collaborates across the EHR system with other rehab professionals. Assists leadership with establishing policies and procedures related to clinical practice. Collaborates with leadership on a regular basis regarding performance improvement to ensure high quality outcomes and participates in agency improvement activities including rehab practice. Assist QA with OASIS audits and activities as needed. Conduct joint visits to assess OASIS, clinical practice and infection control accuracy. Assist with coverage / float in various job functions as determined in collaboration with leadership (e.g. triage, staff development, clinical patient care) as needed. Performs the responsibilities of Licensed Occupational or Physical Therapist or Speech Language Pathology as required in that position profile. Meets agency obligations for scheduling, on-call, weekend, holiday and evening coverage, and expected productivity standards. Performs other duties as assigned by Manager / Director.
