Job Description

General Purpose of Role:  

Ensures effective prioritization of tasks to create bed capacity and maximize patient throughput while mitigating barriers and minimizing negative patient outcomes.

Essential Duties:

This job description is intended to cover the minimum essential duties assigned on a regular basis.  Caregivers may be asked to perform additional duties as assigned by their leader. Leadership has the right to alter or modify the duties of the position.

  • In collaboration with Department Managers, Directors, Staffing Office, and Patient Placement, supervises the work of charge nurses, and ADM's to ensure that departments are facilitating throughput and bed capacity
  • Monitors Patient Throughput by rounding on the nursing units, ED, PACU, Cath Lab and any other areas of concerns. Facilitate removing barriers
  • Represents Hospital Administration and interprets hospital policies and procedures to caregivers, patients, and visitors, investigates and resolves complaints from caregivers, patients, patients' families, and physicians
  • Assists during emergencies by:  Serves as Incident Commander until an administrator arrives, disseminates available resources for direct patient care, obtains emergency supplies as necessary, works with Patient Placement to place patients, communicates appropriate information with caregivers, visitors and patients
  • Responsible for such personnel activities as documenting employee performance, evaluating behavior inconsistent with Hospital values, policies and procedures, and reporting to appropriate manager
  • Acts as a resource person to nursing caregivers to facilitate problem-solving related to nursing care and interpersonal relationships
  • Serves as a role model for consistent demonstration of Sparrow Health System's Customer Service Behavioral Standards for Performance, by respecting the Privacy and Confidentiality of those we serve
  • Demonstrates knowledge and respects patient, service provider, and organizational confidentiality and HIPAA Security procedures and protocols as defined under the HIPAA Privacy and Security Procedures
  • Follows established HIPAA privacy procedures when using/or disclosing protected health information
  • Maintains and protects patient rights under HIPAA Privacy Standards
  • Maintains knowledge of current trends in nursing
  • Keeps senior leadership informed of unusual situations
  • Identifying Discharges:
    • Uses technology, line of sight and verbal communication to identify immediate, short term and next day hospital discharges
    • Participates/Facilitates Throughput Rounds, assigning individual action items to appropriate parties with clear expectations and follows up to ensure execution
    • Provides guidance and resolution to discharge barriers by facilitating escalation to senior leadership for real-time intervention
    • Rounds on IP units, prioritizing by LOC demand, identifying and facilitating patients that are appropriate for discharge lounge
      • Ensures Transport and EVS referrals are placed immediately
      • When appropriate, and per policy, identifies patients that can continue treatment safely in hallway and assigns room to EDI patient
  • Finding Beds:
    • Triages critical bed needs based on admission level of care requirements
      • Reassesses patients throughout their EDI stay for potential upgrades or downgrades
    • Physically rounds frequently on all units to visually identify available and staffed beds
      • Prioritizes IP rounding based on demand
      • Assesses staff capacity and collaborates with unit management to safely take additional admissions
      • Communicates with assigned nurse, charge nurse or ADM when undocumented/unoccupied bed is discovered
        • Initiates expedited EVS order and concurrently updates bed status in EPIC.
        • Documents and reports purposeful deceptive practices to nursing director for PI opportunities
    • Leading Throughput Huddle:
      • Convenes and leads AM bed huddles
        • Facilitates additional noon and afternoon huddles as needed
      • Active participation in daily bed meetings and follows up on action log
      • Escalates unresolved barriers to leadership for intervention
    • Inter-Departmental Collaboration/Reporting:
      • Communicates with Leadership, Case Management, One Call, Patient Placement, all inpatient and outpatient units about placements and anticipated needs
      • Reports on throughput-related metrics:
        • Discharge Exit by 1pm:
        • Admit time < 60 min
        • Discharge Process time: < 90 min
        • Bed Turnover Time: < 60 min
        • 8am EDI\u2019s < 5
        • % of EDI\u2019s > 360 min
        • Monthly Opportunity Day reduction percentage
        • Discharge Lounge occupancy
    • Escalation/Barrier Resolution:
      • Escalates or empowers others to use the chain of resolution to resolve discharge barriers
    • Managing House:
      • Serves as a conduit for all physicians admitting patients
      • Oversees placement of admitted and transferred patients in beds
    • Performs additional duties as assigned
