JOB SUMMARY: This position has primary responsibility for providing Intensive Care Coordination and Intensive Home-Based Services to eligible youth and their families referred through the Flexible Integrated Treatment (FIT) program.  This position serves as the Intensive Care Coordinator for eligible youth. 

MINIMUM QUALIFICATIONS

Education / Experience

Associate's degree plus six years of experience in a mental health field OR

Bachelor's degree in a Mental Health Field plus four years of experience in a mental health field OR

Master's degree in a Mental Health Field plus two years of experience in a mental health field

License or Certification

Valid California Driver's License

Meets Agency driver eligibility criteria

SPECIALIZED SKILLS

Ability to interact and communicate both verbally and in writing with participants, families, schools, other staff, physicians, county agencies, other health care and social services providers and the public. Ability to work independently and as a member of a treatment team. Ability to develop, maintain, and adjust a daily/weekly work schedule that accommodates participants' needs and availability. Good coordination and meeting facilitation skills. Ability to develop strong relationships with other agencies and community partners. Good organizational, writing, and time management skills. Demonstrated understanding of psychiatric illness and its treatment. Ability to understand and work with participants from diverse cultural backgrounds in a positive manner Demonstrated understanding and knowledge of county and community-based agencies

PHYSICAL REQUIREMENTS

Ability to physically perform ProACT. Ability to visually and audibly assess clients' behavior and needs. Ability to stand and walk for long periods, up to 3 hours at a time. Ability to drive within the community in personal and company vehicles

POTENTIAL JOB HAZARDS

Potential for eye and wrist strain from computer screen and minor lacerations from paper cuts. Interaction with potentially assaultive youth.

DUTIES AND RESPONSIBILITIES

Essential Duties

Acts as the Intensive Care Coordinator for eligible youth. Coordinates and facilitates Child and Family Team (CFT) meetings. Provides Intensive Care Coordination (ICC) and Intensive Home-Based Services (IHBS) as identified through CFT meetings. Follows up on CFT recommendations to ensure plans are implemented. Provides services in various settings, including homes, schools, and community and office settings. Uses personal vehicle to drive to and from offsite locations to provide services.  May be required to transport clients and/or client family members in personal vehicle. Participates in meetings, such as Treatment Team Meetings and IEPs, to ensure clients' needs are met and recommendations are followed through. Documents all services provided in a clear and concise manner within the Electronic Medical Record within the timelines and framework established by Capital Star.

Other Responsibilities

Engages with all clients and families throughout services. Assists the primary therapist with initial and ongoing assessments of the client and family to identify strengths and needs. Monitors the clients' progress toward goals outlined in their treatment plans. Informs primary therapist of progress. Develops and maintains relationships with child welfare, behavioral health, and other external partners. May provide mental health services, including individual and group rehabilitation and collateral, and case management to clients and their families, as deemed necessary and appropriate through the assessment process and in collaboration with the treatment team. Participates in TQM activities as requested including CQI subcommittees, probes, peer review, audit and/or tracer activities as appropriate. This job description is not intended to be all-inclusive and employee will also perform other reasonably related duties as assigned by the Clinical Supervisor or his/her designee as needed.

Demonstrated understanding of how to behave in a safe manner while seeing clients in the community or facility as applicable. Demonstrated ability to recognize dangerous behavior by clients and intervene as appropriate.
