Position Summary: A Social Worker, BSW identifies patients potentially requiring long-term care, nursing home placement, rehabilitation placement, foster care, assisted living, home health, hospice, or palliative care services within 48 working hours of admission.  Interviews patients, families, providers, and/or significant others to obtain historical and current information and accurately and clearly records in the Medical Record.  Acts as an advocate for patient within the hospital and community systems striving for excellence in patient care and discharge planning by initiating referrals to other hospital services and/or community resources for those patients returning home.  Consults with care coordinators and unit nursing staff in daily care management rounds for the early identification of patients who nursing requirements suggest the need for specialized planning for post-hospital care and collaborates effectively with the patient's case manager.  Assesses and develops appropriate and safe discharge plans that meet the environmental, psychological and/or socioeconomic needs of the patient and family system.  Conducts case review and discussion with physicians, nursing staff and other hospital or community program staff to monitor patient progress, appropriate discharge planning, and continuity of care under the supervision of an LGSW, LCSW, or LCSW-C.

Education: Bachelor's degree in Social Work from an accredited school required.

Licensure/Certification: LBSW from State of MD required.

Experience: Experience in the medical field is preferred.

Skills: Demonstrated capacity to handle a rapidly changing case load without incurring significant service delays. Ability to facilitate collaborate effectively. Demonstrated ability to process written/electronic applications with corresponding medical information for long-term care placements.

 

MAY BE REQUIRED TO WORK ADDITIONAL HOURS OR VARYING SHIFTS TO MEET THE OPERATIONAL NEEDS OF THE DEPARTMENT.
