Position Summary: The Intake Specialist is responsible for maintaining the remote patient monitoring program (RPM) under the direction of the Outpatient Care Coordination Manager. S/he is responsible for receiving and processing referrals for the program, responding to RPM alerts based on approved protocols and clinical judgment, identifying social determinants of health needs and referring to community services as needed. S/he will be responsible for providing disease specific patient education and self-management education for diseases such as, but not limited to, congestive heart failure, diabetes, pulmonary disease, or sepsis. Patient education may require face to face interaction in the acute care or outpatient care settings and/or in the form of follow up phone calls. S/he is responsible for collecting, analyzing and reporting data related to the RPM program monthly and as necessary. S/he will directly coordinate care with clinical providers, community care coordination, homecare or other members of the health care team based on clinical data review.
Minimum Education: Graduate from an accredited LPN Practical Nursing Program required.
Required License/Certifications: LPN - Nurse LPN License in State of Maryland required. BLS-Basic Life Support certification required, but will provide training if necessary.
Minimum Work Experience: Previous experience as a LPN in an urgent care setting and/or physician's office is desirable.
Required Skills: Must possess good communication skills and demonstrate ability to function independently.
