POSITION SUMMARY:

The Student Care Coordinator at Delta College is entrusted with providing comprehensive support services to students, aiming to bolster their overall well-being and academic achievements. Central to this role is the task of tending to a spectrum of student needs, encompassing non-academic areas such as housing, food security, mental health, financial aid, and access to community resources. Moreover, the Student Care Coordinator assumes the critical responsibility of formulating and guiding a dedicated Student Care Team comprised of social worker interns and allied support staff, all committed to addressing students' holistic needs. This role entails close collaboration with various campus departments, faculty, staff, and external agencies, ensuring that students benefit from prompt and effective assistance. This position reports to the Associate Dean of Retention housed within the Personal Counseling Office and will offer case management services to students of concern through interventions, referrals, and connection to follow-up services.

GENERAL OFFICE AND TEAM CONTRIBUTIONS

  • Embed the College's mission and values throughout all responsibilities and work; and focus on student success as the guiding principle to informed decision-making.
  • Comprehend and maintain current information concerning all programs and student services (Financial Aid, Satisfactory Academic Progress, Guided Pathways, CRM Advise, etc.)
  • Understand, develop, and provide an environment that supports diversity, equity, and inclusion, and incorporates cultural competency in the workplace.
  • Consults regularly with relevant departments on campus regarding services, access, and care to maintain strong collaborative relationships with campus partners in Personal Counseling Services, the Office of Disability Resources, the BEDI division, Public Safety and other areas to provide a comprehensive network of support for students in need.
  • Assist in identifying area trends in case management within the college and help to coordinate targeted and timely response to any identified needs for support and outreach.
  • Contribute to positive teamwork environment.

DIRECT STUDENT SERVICE AND SUPPORT
  • Serves as the primary resource for managing student referrals and support services related to Basic Need insecurities.
  • Establish and Coordinate the Bachelor's and Master's Social Work internship program to serve on the Student Care Team, to address students' diverse needs effectively.
  • Assume the lead Case Manager role for all student referrals initiated through the CRM Case Management System.
  • Offer personalized support and guidance to students navigating challenges related to basic needs, mental health, personal crises, and obstacles hindering academic progress.
  • Develop and maintain a database of referral sources for social service agencies within the local community; facilitate relationships with off-campus agencies and resources to connect students to long-term support.
  • Conduct thorough assessments of student needs and devise tailored care plans to address identified issues and aspirations.
  • Serve as the primary liaison for students seeking assistance and referrals to both on-campus and community resources.
  • Collaborate with relevant campus departments, external entities, and campus stakeholders to facilitate students' access to housing, food aid, financial support, healthcare services, counseling, and other vital resources, while also identifying systemic barriers impeding student success and advocating for institutional policies fostering equity and inclusivity.
  • Oversee all Basic Needs Referrals via the Student Support System.
  • Administer SNAP benefits approval.
  • Maintains CRM Advise Case Management records to ensure all correspondence, notes, action items and rationale are accurately documented. Uphold confidential student records, meticulously documenting services rendered, referrals extended, and outcomes achieved.
  • Develop and execute initiatives, workshops, and outreach programs aimed at fostering student well-being, resilience, and self-advocacy skills.
  • Engage actively in multidisciplinary teams, committees, and task forces dedicated to student retention, crisis intervention, and emergency response.
  • Under the direction of the Dean of Student Success, support students through approved Leave of Absences.
  • Remain abreast of prevailing trends, best practices, and policies concerning student basic needs and social services, informing program refinement and innovation.
  • Participate actively in professional development activities, training sessions, and conferences to enhance proficiency in student support services.
  • Maintain high levels of student satisfaction as measured by student retention, student satisfaction surveys, student graduation rates and graduation surveys.
  • Participate in the process of regularly assessing and evaluating impact of support services relative to student persistence and success.
  • Works with academic deans, program coordinators and faculty regarding individual students and behaviors, which affect the classroom environment.
  • Responsible for leading evidence-based training and assessment that addresses contemporary concerns in a college population and other educational initiatives for faculty and staff regarding awareness of and reporting options for Basic Needs, in collaboration with appropriate campus partners.
  • Assists with maintaining the current directory of community resources available to students, website updates and promotional content regularly.

SKILLS & KNOWLEDGE REQUIRED
  • Strong understanding of issues related to student basic needs, social determinants of health, and equity in higher education.
  • Experience in program development, implementation, and evaluation.
  • Ability to work collaboratively with diverse student populations, faculty, staff, and community partners.
  • Skilled at connecting and building rapport with students from diverse backgrounds.
  • Skilled in navigating difficult and emotionally sensitive conversations with others.
  • Outstanding interpersonal, oral, written communication and oral resolution skills; excellent customer service; exceptional organization and time management skills.
  • Self-motivated, organized, innovative, solution seeking, with a desire to encourage others to succeed.
  • Knowledge of local community resources and support services for students.
  • Commitment to promoting diversity, equity, and inclusion in student support services.
  • Proficiency in Microsoft Office Suite and other relevant software applications.
  • The Student Care Specialist may be required to work evenings and weekends occasionally based on student needs and programmatic activities.
  • Master's degree in Social Work
  • Two years of experience in a related area
  • Experience in case management including demonstrated documentation and tracking skills, follow-up on projects and assignments.
  • Ability to work cooperatively with others within a multidisciplinary team environment, evidence of effective collaborative relationships with students, faculty, and staff.
  • Licensed Professional Social Worker.
  • Eligibility to supervise Bachelor or Master's social work interns.
  • Previous experience in working with at-risk individuals.
