The Home Health Community Liaison is a marketing/relationship building and educational position in which the Liaison will act as a consultant to our referral sources and communities, providing them with education on Home Health services and other programs that our agency offers. You will interact with leads and convert leads into home health referrals while at the same time, working to build Discovery at Home's patient's census as well as the community awareness of who we are. In this role, you are an important component to the success of our agency and the communities that we service.  Our goal is to grow!

Responsibilities include:

  • Successfully communicate and create community/industry awareness around Discovery at Home's services for both Medicare Skilled services and Non-Skilled Private Pay services.
  • Take inbound calls, follow up on leads and referrals from family members, caregivers, and other sources while helping to guide families with their care needs starting at the initial consultation
  • Make the lives of our families and customers better, easier and more successful by assisting in coordination of referrals post hospitalization or rehab admission
  • Build relationships with community team members as well as various referral sources while acting with integrity and assuming positive intent
  • Focus on excellence and driving outcomes every day with passion and a sense of mission
  • Attending weekly meetings within the communities we serve as a representative of Discovery at Home
  • Collaborates with home office as well as interdisciplinary team on a daily basis to aide in better services our patients and referral sources.

Qualifications:

  • Clinical license preferred (LPN, PTA, COTA) but not required
  • 1-year home health experience required
  • Enjoys working with the senior population
  • Strong verbal and written communication skills
  • Excellent telephone etiquette
  • Consummate people skills. Demonstrates talent at interacting effectively with all types of people
  • Ability to use Microsoft Windows, including Word, Outlook and Excel. Ability to use email and the Internet. 
  • Proficient organizational skills and must be detail oriented
  • Ability to solve problems
  • Can take on responsibilities and manage priorities with minimal guidance
  • Projects a professional and polished image that inspires confidence and trust
