• Provide therapy according to the PT's established plan of care
• Help residents improve mobility, strength, balance, and independence
• Monitor progress and communicate changes to the supervising PT
• Educate residents and caregivers
• Collaborate with the community and clinical teams
• Complete accurate and timely documentation

• Graduate of an accredited Physical Therapist Assistant program
• Current state PTA license
• Current CPR certification
• Strong communication and documentation skills
