Registered Nurse (RN) | Private Duty Home Health | PRN Flexible nursing. Personal care. Meaningful relationships.
At Discovery at Home, our Private Duty nurses have the opportunity to provide personalized care while building meaningful relationships with the patients and families they serve.
This is a PRN position that requires participation in an on-call rotation. It's a great fit for an experienced home health RN looking for flexibility, competitive per-visit pay, and the support of an established clinical team.

- Recertification: $80
- Routine Visit: $55
- Provide skilled nursing care in the patient's home
- Complete assessments and individualized plans of care
- Administer medications and treatments as ordered
- Monitor changes in patient condition
- Educate patients and families on medications, treatments, and care needs
- Coordinate care with physicians and other members of the care team
- Complete accurate and timely clinical documentation
- Participate in the PRN on-call rotation
- Current, unrestricted RN license
- ADN or BSN from an accredited nursing program
- One year of home health experience required
- Current CPR certification
- Knowledge of Medicare and Medicaid guidelines
- Valid driver's license and reliable transportation
- Strong clinical assessment and communication skills
