What You'll Do

• Provide skilled nursing care to residents receiving home health services
• Complete assessments and develop individualized plans of care
• Administer medications and treatments as ordered
• Monitor resident progress and changes in condition
• Educate residents and families on medications, disease management, and care needs
• Coordinate care with physicians, therapists, caregivers, and community teams
• Complete accurate and timely clinical documentation

What You'll Need

• Current, unrestricted RN license
• ADN or BSN from an accredited nursing program
• Home health experience required
• Current CPR certification
• Knowledge of Medicare and Medicaid guidelines
• Valid driver's license and reliable transportation
• Strong assessment, communication, and documentation skills
