Per Diem | Bergen County, NJ | 10 minutes from NYC

 

Englewood Health is seeking a Patient Blood Management (PBM) Navigator to play a key role in optimizing patient outcomes by reducing unnecessary transfusions, improving anemia management, and coordinating evidence‑based strategies across the continuum of care. The advanced practice provider will collaborate with surgeons, anesthesiologists, hospitalists, nursing, laboratory services and quality teams to ensure safe, efficient, and patient‑centered, blood management practices.

The ideal candidate is clinically strong, detail‑oriented, and passionate about improving quality, and reducing variation in care.

 

About the Opportunity

 

  • Schedule: Up to 15 hours per month.
  • Bilingual ability preferred (Spanish/Korean).
  • Ability to travel with a current driver's license and access to a vehicle.

 

What your day will look like

 

  • Coordinate and manage the hospital's Patient Blood Management program in alignment with national standards and accreditation requirements.
  • Identify patients at risk for anemia or transfusion and facilitate early intervention, including preoperative optimization and outpatient follow‑up.
  • Collaborate with multidisciplinary teams to develop individualized PBM care plans.
  • Monitor transfusion practices and support adherence to clinical guidelines.
  • Educate clinicians, staff, and patients on PBM principles, anemia management, and transfusion alternatives.
  • Track, analyze, and report PBM metrics, trends, and outcomes to leadership and quality committees.
  • Support implementation of evidence‑based protocols, order sets, and clinical pathways.
  • Serve as a liaison between clinical teams, laboratory services, and blood bank operations.
  • Participate in performance improvement initiatives and regulatory readiness activities.
  • Maintain documentation and ensure compliance with hospital policies and external standards (e.g., AABB, Joint Commission).

 

Qualifications

 

All Candidates

 

  • Minimum 3 years of clinical experience in acute care, perioperative services, hematology/oncology, critical care, or transfusion services
  • DEA and NJ CDS registration
  • Current BLS and ACLS certification

 

Advanced Practice Nurse

 

  • Master's or Doctorage degree in Nursing from an accredited program
  • National Board certificatioin is required
  • Current NJ Professional Nurse License
  • Current NJ Nurse Practitioner License

 

Physician Assistant

 

  • Master's degree from an accredited Physician Assistant program
  • Current NJ Physician Assistant License
  • Current National Commission on Certification of Physician Assistants (NCCPA) Certification

Englewood Health offers competitive total compensation that includes base pay, comprehensive benefits package, and other incentive programs for our employees. The rate shown reflects the anticipated hourly wage range for this position; actual pay will depend on several factors, including educational background, skills, and professional experience.

Pay Rate: $55.00 per hour
