Hackensack Meridian Health is hiring for a Social Worker to support our growing team at the Center for Memory Loss and Brain Health. In this role, you will collaborate with nurse navigators, physicians, medical asssistants, and other members of the healthcare team to ensure appropriate utilization of resources, care planning, health education, patient satisfaction and revenue recovery.
This is a full-time, on-site, day shift position. This team will be transitioning to an office in Paramus, New Jersey in Summer 2024.
Education, Knowledge, Skills and Abilities Required:
- Master's degree in Social Work (MSW).
- Minimum of 1 years of prior experience as a Masters level Social Worker in a healthcare setting.
- Minimum of 1 year of experience working with an EMR, specifically EPIC.
- Ability to work with diverse patient and team member/staff populations.
- Strong computer communication,analytical and presentation skills.
- Ability to collect, manage and analyze data.
- Experience with Microsoft Word, Excel, Outlook, PowerPoint, Google.
Education, Knowledge, Skills and Abilities Preferred:
- Spanish speaking.
- Certification in data analysis or business intelligence (BI).
Licenses and Certifications Required:
- NJ Licensed Social Worker.
