Holyoke Medical Center is an independent community hospital that has served the healthcare needs of the Pioneer Valley for over 125 years.# We continue to grow and address the needs of our community through quality and patient- centered care. The CHW provides chronic diseasea education, smoking cessation counseling and links patients to mental health resources and other social determinants of health. We are seeking a Community Health Worker who, under the general direction of the Senior Community Navigator and Manager, collaboratively provides comprehensive care management services in response to e-referrals, and assists patients/clients to navigate and access community services that will have a positive impact on their overall health. This position requires a non-smoker for at least 2 years . REQUIREMENTS: High School Diploma or equivalent; bi-lingual or multi-lingual preferred; driver#s license and vehicle; must complete smoking counselor training within one year of hire; completion of#CHW Core Competency Education in Massachusetts; trained and certified in chronic disease management such as asthma, hypertension, diabetes and mental health; pediatric experience a plus Our competitive salary package is adjusted to align with a candidate#s relevant experience and skills.
