We continue to grow and address the needs of our community through quality and patient-centered care.# Our Orthopedics Surgeons Practice is seeking a Nurse Navigator to coordinate patient care across the healthcare continuum, providing education, self-management support, and timely access to services to promote optimal health outcomes. The Nurse Navigator assesses patient needs, identifies and addresses barriers, facilitates communication, and evaluates outcomes to continuously improve the quality of care. Overview of responsibilities include but are not limited to: Collaborates with physicians to identify patients appropriate for complex care management based on established criteria. Conducts comprehensive initial and ongoing assessments to identify patient needs, prioritize care, and determine appropriate follow-up. Promotes patient and family education, self-management, and healthy lifestyle choices to support wellness and independence. Coordinates care with physicians, healthcare teams, facilities, payers, patients, families, and health system partners across the continuum. Identifies and addresses barriers to care and facilitates appropriate referrals and services. Provides patient education and navigation related to healthcare procedures, treatments, and care plans. Maintains accurate, timely, and comprehensive clinical documentation. Uses electronic tools, reports, and other resources to support care management, prioritization, and decision-making. Performs duties in accordance with the Nurse Practice Act, professional nursing standards, and organizational policies. Maintains knowledge of and compliance with applicable federal and state regulations, including patient confidentiality and release-of-information requirements. Qualifications: Education:#Graduate of an accredited school of nursing, Bachelor#s degree preferred Licenses/Registrations:#Current RN license within the Commonwealth of Massachusetts; Valid#Massachusetts Driver#s License # Experience:#Three to five years of clinical-related job experience Special Aptitudes # Activities:#Strong written, verbal, and interpersonal communication skills with the ability to build effective working relationships with patients, providers, staff, and internal and external partners. Demonstrates strong organizational and prioritization skills, attention to detail, and the ability to work independently while effectively collaborating with a team.
