The Holyoke Visiting Nurses Association, Inc. is looking to hire a #Hospice On-Call RN, which is a benefited position with a $10,000 sign on bonus for experience! As a successful and growing agency, Holyoke VNA # Hospice Life Care is looking for an experienced RN who wants to service our community in an evening and on-call capacity.# We are looking for a compassionate detail-oriented nurse to provide care#to our patients. This is a benefited position.# Benefited Hospice Evening and On-call RN coverage will be provided between the hours of 4:30PM and 8:00AM Monday through Saturday starting at 4:30pm and until 8AM, and any other time the office is closed to assure appropriate follow-up at all times to hospice patients.# The on-call RN will begin their shift Monday at 4:30pm and they will get off call Saturday at 8am (with the exception of not being on-call during operating hours 8AM-4:30pm). The evening and on-call position does not include working weekends; it is strictly 4:30pm-8:00am Monday through Saturday. When patient calls come in after 4:30PM, the answering service will pass on every call to the on-call nurse. In addition it also includes working 2 holidays on a rotating basis. The on-call RN will provide the follow appropriate responsibilities/tasks to the call and will make a home visit if one is needed. Here is the breakdown for the Hospice Evening and On Call RN position with specific hours: 1.) Monday- Friday 4:30pm to 11:30pm (6.5 hours x 5 = 32.5 hours) Hospice On Call Office every night between 4:30pm to 11:30pm, with a 30 minute meal break. You will make scheduled evening visits and will#log any visits or phone calls that are made during this time and document travel time. This is considered regular time, not #on call time#.##Include mileage during this time for visits that are #unscheduled# or based on a call to the answering service. Consider any scheduled visits or admissions after hours as starting from the office. Include mileage in between visits when there is more than one visit being made. 2.) Monday- Friday 11:30pm- 8:00am (8.5 hours x 5 =42.5) This is considered on call time. Any visits, calls or travel time is paid as #on call# time# and you will be paid your regularly hourly rate. If you have any early morning visit or phone call that goes over 8am, you will be paid additionally for this time at your hourly rate.# This position accrues#vacation and sick pay based on 32.5 hours/ week. Holidays are pro-rated based on 32.5 hours/week.# # Responsibilities may include any and all of the following: Admissions, symptom management visits and/or pronouncement visits. Instruct patients and caregivers in symptom management and end of life care. Communicates with MD to facilitate recommendations for symptoms and pain management. Evaluate outcomes of nursing intervention and initiate changes when appropriate Collaborate, communicate and cooperate as appropriate with other health care providers to ensure quality and continuity of care.
