The Holyoke Visiting Nurses Association, Inc. is looking to hire a Palliative Registered Nurse Full Time at either 32 or 40 hours. We are offering a $5,000 sign on bonus for this position to an experienced Registered Nurse in the following areas: Home Care, Palliative and Hospice.# This is a#great opportunity to grow with the agency, as our program continues to expand and develop!! Some of the Essential Job Functions include the following: Assume responsibility and accountability for the practice of professional nursing in accordance with the State Nurse Practice Act and standards for community/home health nursing. Perform comprehensive assessments of patient status, including physical, psychosocial, cultural and environmental parameters. Develop and implement individualized plans of care that incorporate analysis of data and current best practice guidelines. Coping strategies, support system, and learning needs of client/caregivers. Provide skilled interventions aimed at achieving realistic patient outcomes within a specified time period. Report changes and information necessary to modify and update the care plan to reflect progress towards goals. Supervise applicable home care team members.# Perform in home supervision of Home Care Aides according to Agency policy. Perform patient assessments based on Agency standards of practice to ensure effective and appropriate home care services. Apply standard nursing principles and practices to utilize a holistic approach to patient care. Consider the physical, psychological, and socioeconomic needs of the patient, the level of family acceptance, and the ability for patient needs to be met in the home. Collaborate with physicians, other health care professional (e.g. therapists, social services, supportive services), and patients/families in developing a comprehensive, quality plan of care. Initiate referrals as needed to other health team members and/or community resources. Analyze the assessment data and work with the Interdisciplinary Group to: Identify problems that may be resolved, diminished or prevented through nursing and/or interdisciplinary intervention; Utilize the team approach to improve Palliative management of patients. Direct, plan and initiate appropriate action independently and responsively in home care situations. Observe for and identify overt and subtle signs of impending physiologic and psychological changes.# Notify the physician in accordance with Agency policies and procedures. Provides direct skilled nursing care and emotional support. Make decisions and/or recommendations that reflect consideration of immediate and long-range effects (frequency of visits, components of care plan, additional services). Confer with the IDG on a regular basis.# Utilize Agency and community resources appropriately. Document in clinical record to ensure compliance with regulatory standards of timeliness, accuracy and completeness.# Document legibly. Provide on-call support for patients based on needs of the Agency.# (Rotate to weekends, evenings, and holidays to maintain adequate staffing.) Communicate with other disciplines/departments as appropriate. Consistently demonstrate competency with skills according to professional and legal scope of practice. Promote personal safety and a safe environment for patients, families and team members. Demonstrate knowledge of safety and infection control practices by compliance with Agency policies and procedures. Recognize and respond appropriately to potentially unsafe situations. Demonstrate safe and competent practice in the use of equipment. Assess safety of the environment and take initiative to prevent accidents and promote safety. Participate in all mandatory safety education per Agency policy. Maintain established agency, hospital and departmental policies and procedures, objectives, confidentiality, quality improvement program, compliance, safety, infection control and environmental standards. Maintain professional growth and development through attendance at seminars, workshops, conferences or in-services, professional affiliations or journals to stay abreast of current trends in field of expertise. Meet annual competency and retraining requirements.# Maintain any certifications required for this position. Attend meetings as required. Perform other functions/duties as requested. MINIMUM REQUIREMENTS: #BSN preferred. Current license to practice as a Registered Nurse in the Commonwealth of Massachusetts required.# Current CPR certification required.#Minimum of one (1) year of medical /surgical nursing experience required; palliative care/hospice experience and certification preferred.# Demonstrated knowledge of home health regulatory and reimbursement requirements preferred. Demonstrated excellent verbal and written communication skills and strong interpersonal skills required.# Demonstrated ability to engage in discussions with patients/families about serious illness, quality of life and functional well-being. Demonstrated organizational and time management skills required. Demonstrated knowledge and understanding of chronic disease case management with an ability to identify and treat signs and symptoms of the same including competency in pain and symptom management required.
