The Referral Coordinator at Keck Medicine of USC is a motivated, organized and efficient individual who has responsibility for managing and coordinating the process of referring patients to the appropriate provider, communicating back to the patients with the appropriate information, supporting the clinic in synchronizing appointments, authorizations and tests taking place in various clinics. . The position requires good telephone and interpersonal skills, efficient organization of incoming consults, and ability to coordinate team communication throughout the day. The position also requires a proficient knowledge of computer software applications including Word, Excel and Access for the purpose of creating spreadsheets, maintaining databases and contributing to the performance of routine analyses of the Excel Data. Abilities with PowerPoint also a plus. The position will require the person to self-direct for data input and prioritization of tasks for clinical issues and scheduled data deadlines.
The Healthcare Referral Coordinator for Charity Care & Community Resources will play a pivotal role in assisting the Care Coordination team of a tertiary hospital. This individual will be responsible for facilitating patient referrals to charity care and private resources offering financial assistance to individuals and families facing hardship due to hospitalization or long-term/catastrophic illness. The coordinator will also identify and connect patients with community programs and services that may provide additional support in specific situations. This position requires strong organizational skills, the ability to work cross-functionally with various hospital departments, and the ability to build relationships with local community organizations.
Essential Duties:
- Management of The Change Reaction Angel Fund that provides renewable $100,000 grants for the purpose of distribution to patients / families with demonstrated need. The funds are available as emergency funds for individuals and / families in situational crisis (DME, car repairs, transportation, utility bills, housing costs or child care); One-time rental assistance required to secure/maintain an adequate living situation. o Assess each referral including: Name of requestor Nature of relationship to Grantee (employee, patient, family) Amount of the request Brief description of situation o Obtain an authorization to release information. o Enter information into the portal and monitor for approval / declination. o Inform referring social worker of status of requests. o Prepare Award Letters. o Work with Social Work Director to distribute funds.
- Manage Accounting of Angel Fund distributions: o Number of grants made; o Dollar amount of each grant made; o Total amount of the Fund drawn down in the reporting period; o Total amount of the Fund remaining; and o Brief narrative description of each grant made.
- Patient Referrals to Charity Care: o Assess patient needs and facilitate referrals to available charity care programs, financial assistance resources, and other related services. o Coordinate with insurance and billing departments to explore available options for patients who are uninsured or underinsured. o Maintain an up-to-date knowledge of local, state, and national charity resources and their eligibility requirements.
- Identifying Community Programs and Services: o Proactively research and identify local and regional community programs and services that may provide financial or logistical assistance to patients and families dealing with medical crises, including transportation, housing, food, and utilities. o Establish and maintain partnerships with local community organizations, charitable foundations, and other non-profit agencies. o Ensure referrals to community resources are timely and appropriate for patient needs.
- Data Entry and Documentation: o Maintain accurate, confidential, and up-to-date records on all patient referrals to charity and community resources in compliance with hospital documentation standards. o Track and report on the utilization and outcomes of referrals to charity and community resources.
- Collaboration with Hospital Care Teams: o Work closely with the Care Coordination team, physicians, social workers, and other multidisciplinary team members to ensure a holistic and patient-centered approach to care. o Participate in interdisciplinary meetings to share information about available resources and provide feedback on patient needs and care outcomes.
- Compliance and Privacy: o Ensure all patient information is handled in compliance with HIPAA and hospital confidentiality standards. o Keep abreast of changes in healthcare regulations regarding charity care, financial assistance, and community resource programs.

Required Qualifications:
- Req High School or equivalent
- Req Strong organizational and time-management skills with attention to detail.

- Req Ability to build and maintain relationships with external community organizations, patients, and multidisciplinary team members.
- Req Proficiency with healthcare software and Microsoft Office Suite.

- Req Strong problem-solving skills and the ability to think critically in identifying resources to address patient needs.
Preferred Qualifications:
- Pref Bachelor's Degree Social Work, Healthcare Administration, Nursing, or a degree in a related field.
- Pref 2 years general healthcare experience in a clinical or hospital setting, preferably with exposure to social work, care coordination, or case management.
- Pref Experience with community resource identification and referrals is highly desirable.
- Pref Certification - Job Relevant Notary Public Certification
Required Licenses/Certifications:
- Req Fire Life Safety Training (LA City) If no card upon hire, one must be obtained within 30 days of hire and maintained by renewal before expiration date. (Required within LA City only)
The hourly rate range for this position is $25.00 - $39.69.
