The Medical Record/Health Information Technician is responsible for the care of the patient health record to ensure that the information in the record is complete, accurate, and only available to authorized Caregivers and requestors.
Essential Job Functions:
The Medical Record/Health Information Technician:
Assists Caregivers and Providers (physicians, clinical departments, advance practice nurses) with service needs.
Completes the identification of documents which constitute the legal content of the clinic, hospital outpatient, and hospital inpatient patient record, in both electronic and paper formats.
Scans paper health records from internal and external sources and accurately identifies the accurate target location of the document (indexes or assigns form ID) for ease of review for continuing health care and internal and external operations.
Evaluates the completion of the record utilizing medical records practice standards for clinics, critical access hospitals, acute care hospitals, and long term care facilities.
Receives and validates telephone, verbal, and written requests for information with health care providers, authorized third parties, patients, or their authorized representatives requesting patient information for purposes of continuing care, payment, or internal or external operations.
Documents the receipt of and completion of all requests for information.
Capably utilizes health care software and office management software.
Prepares and handles information in several media forms accurately: printing, scanning, copying, faxing, sorting, collating, or filing.
Participates in team meetings with communication skills including active listening, active note taking, and feedback.

The medical record Caregiver accepts flexible and essential assignments which vary according to the requirements of the scheduled goals.
The medical record Caregiver is skilled in general office management skills including professional communication, interacting with co-workers in process review and problem solving using problem oriented procedures.
The medical record Caregiver receives training through verbal and hands on training, and by reviewing and applying standard continuous improvement processes.
The medical record Caregiver operates personal computers and software, telephone, and document scanner.
Associate Degree in Medical Record-Health Information Management
5 years of experience in a large clinic or hospital environment.
High school graduate or equivalent.
1 year of experience in a large clinic or hospital environment.
REQUIRED WORK HOURS: Day shift-Monday through Friday. Office hours are 6 AM to 6 PM.
REQUIREMENTS: Hearing and verbal communication required. Sedentary work including at desk and at computer use constantly. Lifting no more than 25 pounds occasionally. Light walking and reaching.
