Description

Manages and leads teams responsible for navigation and care coordination for federal and/or military related contract participants. Ensures efficient and effective workflows in a collaborative and coordinated response ensuring exemplary service delivery for information/referral, locating appropriate services, arranging access to services and supporting communication in a multi-disciplinary environment. Provides supervision ensuring team members provides coordination of services with application to identified populations using assessment, case planning, implementations, coordination, monitoring and evaluation for cost effective and quality outcomes. Provides initial and ongoing navigation and care coordination training. Ensures compliance with contract and company policies, procedures and practices.

  • Manages, leads and directs navigation and care coordination services ensuring contract, company policy and practice compliance for teams supporting federal and/or military related contracts.

  • Ensures teams maintain current knowledge, research availability of resources, services and links participants to appropriate services.

  • Manage team members` performance through qualitative and quantitative performance following company processes and policy.  

  • Creates a team environment conducive for meeting productivity targets and instrumental in achieving and sustaining high retention rates.

  • Ensures successful delivery of high quality, customer-focused, cost effective and efficient services.

  • Models and demonstrates a highly coordinated and collaborative management style producing a highly effective, productive team.

  • Ensures effective workflows involving information and referral, case management, care coordination, utilization management, and quality improvement.

  • Responsible for reviewing, analyzing, and producing reports within company and contract timelines meeting reporting expectations. 

  • Effectively performs in a highly collaborative, matrixed multi-disciplinary team environment.

  • All other duties as assigned.

Other Job Requirements

Responsibilities

Bachelor's degree required in Human Services, Behavioral Health or related field.

A combination of education and work experience may be considered.

5+ years post bachelor's degree experience in social services, call center, customer service or related area.

2+ years supervisory experience in related areas.

Ability to effectively coach, mentor and develop team members.

Ability to function independently leading a team, analyzing and troubleshooting challenges delivering high quality services.

Demonstrated track record of managing change with proven results in a dynamic environment.

Effective verbal and written communication skills. Ability to work collaboratively and within a matrixed organization.

When required, obtain and maintain required level of a government clearance.

Salary Range

Salary Minimum:

$64,285

Salary Maximum:

$102,855

This information reflects the anticipated base salary range for this position based on current national data. Minimums and maximums may vary based on location. Actual pay will be adjusted based on an individual's skills, experience, education, and other job-related factors permitted by law.
