Description

The Housing Program Specialist connects members with appropriate housing resources by gathering information, researching available programs, and maintaining up-to-date resource files. The specialist ensures accurate communication with members assisting with housing-related needs. They build targeted resource lists, collaborate with community organizations, and stay informed about housing policies and funding changes. Strong organizational, research, and communication skills, along with a commitment to equitable service delivery, are essential for success in this role.

  • Respond to housing referrals by gathering detailed information about the member, including their location and unique needs.
  • Research available housing programs and resources in the member's area and match them to appropriate programs they are eligible for based on the collected information.
  • Report weekly on referrals worked and consult with colleagues about complex situations.
  • Attend rounds to provide housing resources as needed.
  • Provide a curated list of housing resources to members, their Care Coordinators, and other support staff or community members assisting with housing searches.
  • Update and maintain current housing resource files, ensuring all links, phone numbers, staff contacts, and service descriptions are accurate and current.
  • Build specific resource lists as concerns or trends arise, such as creating targeted resource files to address the needs of specific populations, like older adults experiencing houselessness.
  • Attend community events, meetings, and functions to collect additional resources and information about potential changes to relevant programs.

Responsibilities

2+ years' experience in housing or care management.
Positive, recovery-oriented attitude that conveys hope and maintains empathy.
Knowledge of housing resources, programs, and services, including eligibility criteria and application processes.
Familiarity with federal, state, and local housing policies, funding, and procedures.
Experience working with Medicaid populations, including individuals experiencing houselessness, older adults, or those with disabilities.
Strong research and organizational skills to identify, maintain, and update resource databases.
Ability to build and maintain relationships with community organizations, agencies, and service providers.
Excellent communication skills, both written and verbal, to effectively collaborate with members, Care Coordinators, and support teams.
Problem-solving skills to address unique housing challenges and identify appropriate resources.
Proficiency in using technology and the Microsoft Office Suite, including database management, spreadsheets, and communication tools.
Ability to attend and participate in community events, meetings, and functions as a representative of the organization.
Sensitivity to diverse cultural and socioeconomic backgrounds, with a commitment to equitable service delivery.
Strong time management skills and the ability to handle multiple priorities in a fast-paced environment.
Experience in reporting and documentation, including maintaining detailed electronic health records and resource files.
Flexibility to adapt to evolving community needs and organizational priorities.
Must own a reliable vehicle for regular community-based travel.
