About the Job

General Summary of Position

Provide transitional management of patients with chronic conditions within a team-based ambulatory care model. Focus is on optimizing medical management, supporting care transitions, patient education, and continuity between hospital discharge and primary care follow-up. Working closely with primary care providers, community health advocates, and specialty services, the APP coordinates care plans, facilitates timely access to appropriate services, and addresses clinical issues that arise during care transitions.

This position emphasizes collaborative practice, proactive follow-up, and effective communication to improve outcomes for patients with chronic and complex medical social needs. Strong clinical judgment, organizational skills, and the ability to work effectively across disciplines are essential.

Primary Duties and Responsibilities

 

  • Provides comprehensive patient care, including assessment, diagnosis of common acute and chronic conditions, development of treatment plans, and prescription of medications in accordance with practice and hospital policies.
  • Orders and interprets laboratory and diagnostic tests, monitors results, adjusts medications, and communicates findings to patients and the care team to ensure continuity and quality of care.
  • Collaborates with physicians, clinical staff, and community resources to coordinate care, address changes in patient status, and support practice goals.
  • Maintains accurate patient records, conducts interval histories and physical exams, and reviews medical information to assess and document health status.
  • Ensures compliance with departmental policies, safety standards, and applicable federal, state, and local regulations.
  • Provides patient and family education, emotional support, and counseling to promote understanding and adherence to care plans.
  • Acts as a liaison between patients, providers, and specialty services to maintain continuity of care during transitions, including post-hospital follow-up

Minimal Qualifications Education

 

  • Completion of a physician assistant training program accredited by the Accreditation Commission on Education for the Physician Assistant (ARC-PA) AND Current PA-C designation
  • Completion of an accredited nurse practitioner program
  • AND Current NP certification

Experience

  • 1-2 years Related experience; specialty experience may be preferred based on location

 

Additional requirements

 

  • CPR - Cardiac Pulmonary Resuscitation (includes BLS and NRP) for healthcare providers from either the American Heart Association (AHA) or American Red Cross within 90 Days required and

Knowledge Skills and Abilities

  • Demonstrated effective communication and problem-solving skills.

 


This position has a hiring range of

USD $115,003.00 - USD $165,006.00 /Yr.
