General Summary of Position
Incumbent will ensure accurate and compliant deliverables reporting in line with federal, state, and local regulations, including HIPAA, COPPA, and FERPA, while managing program data processes. It involves enforcing standard operating procedures for compliance, supporting staff training, and assisting with logistics and coordination for performance metrics, community organizing, and sub-grantee liaison for reporting and evaluation.


Primary Duties and Responsibilities

  • Responsible for reviewing new grant contracts to extract performance requirements and computing baseline data for required metrics, tracking changes over time to assess outcomes. It also includes creating data dictionaries, identifying data sources, and defining methods for calculating grant deliverables across various program areas, such as Food Rx, Mobile Health Center, and Health Equity Resource Programs.
  • Responsible for developing "goal to actual" comparisons for grant metrics, extracting and reporting data monthly using various platforms like MedConnect, Tableau, and Smartsheet. It includes creating audit trails, ensuring data integrity, assisting with platform design for reportable deliverables, and providing staff training on new data elements, while also supporting community event logistics and partner engagement.
  • Responsible for sub-grantee contracts (community partner contracts, vendor contracts, CCHS grantees) provides technical assistance for accurate and timely completion of their performance reports.
  • Supports performance reporting to funders/regulators. Shares all data reports and analysis with Grants Compliance specialist, Finance compliance Coordinator, and PI/PD during end of month close reviews - for analysis against time and effort shown as salary spent for the period.
  • Supports programmatic success by identifying trends and outliers to improve performance and best practices, including analyzing metrics like productivity, patient engagement, and payer mix. It also includes participating in end-of-month reviews for each grant, using data-driven insights to assess performance compliance.
  • Contributes to the achievement of established department goals and objectives and adheres to department policies, procedures, quality standards and safety standards. Complies with governmental and accreditation regulations.
  • Participates in meetings and on committees and represents the department and hospital, participates in multidisciplinary quality and service improvement teams, as required.
  • May assist Grant Compliance Specialist in the selection, training and orienting of department staff.

    Minimum Qualifications
    Education

    • Bachelor's degree Non-profit management, Public Health required

    Experience

    • 3-4 years Statistical analysis, community relations, project management required and
    • Experience working in medical office, hospital or ambulatory setting preferred

    Licenses and Certifications

    • Portfolio Management Professional (PMP) preferred or
    • Certified Project Management Professional (PMP)-PMI preferred

    Knowledge, Skills, and Abilities

    • Working knowledge of Microsoft Office software and CRM or clinical databases (Social Solutions, Salesforce)
    • Excellent verbal and written communication skills
    • Problem solving, customer service and time management skills
    • Ability to manage PHI and PII documentation


    This position has a hiring range of $58,656 - $98,384
