General Summary of Position

Medstar Health is looking for a Social Work Case Manager to join our team! We are looking for a Licensed Clinical Social Worker (LICSW) with strong written and verbal communication skills and experience managing and presenting data to diverse groups in a professional setting. 

The Social Work Case Manager Licensed Clinical Social Worker (LICSW) manages patients accessing care within any of the MedStar Health hospitals needing support. The position is responsible for coordination of care through assistance with eligibility, resources identification and linkages to care and follow up. This position provides services that engage patients to minimize barriers to care and maximize health outcomes.

Primary Duties: 

  • Consistently demonstrates support for compliance in the performance of job duties and responsibilities by a) developing and maintaining knowledge of, and performing job functions in compliance with, rules, regulations, policies, and statutes that affect his/her job; b) maintaining knowledge of and performing job functions in compliance with MedStar Health and MedStar Health Research Institute policies and procedures; and c) reporting compliance concerns in an appropriate and timely manner.  

  • Actively participates in grant writing proposal activities, following up with participating organizations updating the Memorandums of Understanding with programs, if applicable. Appropriately and effectively documents MedConnect (EMR) in a timely manner and maintain current on-going individualized treatment care plans as outlined by funding source. Collaborates with community providers to identify resources and enhance service delivery.  

  • Assists with the development of self-management curriculum and guides patients towards effective implementation to maximize health outcomes. Coordinates and schedules initial and follow-up appointments with behavioral health providers and provides behavioral health screenings, individual and/or group psychosocial support services. Facilitates transition of care between providers for eligible patients from the program. Collaborates with community providers to identify resources and enhance service delivery through appropriate follow-through and documentation of referrals in the EMR  

  • Guides patients towards effective self-management and sustainability by providing treatment education adherence and re-engagement services. Includes coordination of services with a navigator/advocate, community outreach specialists, home health visiting services, behavioral health care providers and other specific department staff/teams. Interacts with community groups and providers to provide educational programs, talks, outreach, support groups, and successful linkages to care. 

  • Provides medical case management to include adherence with care, harm reduction, education, other medical issues, and psychosocial management. Work with patients to determine eligibility status for medical care, pharmaceutical coverage, housing, or other social issues to eliminate barriers to effective health management. Works across the spectrum of funding sources to manage patients on a continuum of care towards retention or external linkages to meet specific needs. Maintains and enters data weekly as required by program and funders 16. Provides clinical guidance, leadership, and support to non-licensed staff and to LGSW staff, social worker students (if applicable). Review and counter sign notes of those staff. 

 

 

Qualifications: 

  • Master's degree in social work and an advanced clinical license based on work jurisdiction. 

  • 5-7 years as a licensed independent clinical social worker (LICSW).  

  • At least 5 years' experience in a clinical care setting with working knowledge of funding services, electronic medical records, and quality improvement.  

  • Experience with vulnerable populations. 

  • LICSW in respective state of employment.
