Find your calling at Mercy! The Resource Nurse Advanced Primary Care provides virtual care to patients that benefit from ongoing chronic disease management. In partnership with a multidisciplinary team, including physicians and advance practitioners, the Resource RN is responsible for creating patient relationships, monitoring vital sign trends, conducting patient visits in between regular office visits, assisting in panel management of patients and providing individualized care to improve patient outcomes. Reports to respective Advanced Primary Care Lead APP. Position Details:

6. Ability to monitor multiple patients and work within a treatment team that provides patient centric care. 

7. Ability to concentrate for long periods of time, work well under stress and be able to think and react quickly in emergencies. 

Licensure and Certifications 

Is personally responsible for obtaining, and maintaining, a current RN license within the hiring state and/or compact licensure in which nursing duties are performed and must meet all state board of nursing requirements.

Must be eligible for licensure in other states. 

Education and Experience 

Graduate of an accredited registered nursing program 

Bachelor's degree in nursing (BSN) or equivalent work experience 

Minimum 5 years of healthcare experience required.  

Previous telemedicine, internal/family medicine, or care management experience preferred.
