Description
Find your calling at Mercy! Provide skilled nursing care to patients in their homes under physician direction. Responsible for assessing, planning, implementing, and evaluating patient care to support safe recovery and independence. Position Details:
Key Responsibilities
Deliver professional nursing care in the home setting following physician orders
Develop and update individualized care plans with measurable goals
Monitor patient progress and communicate changes to physicians and care team
Educate patients and caregivers on care plans, treatments, and medications
Coordinate services with interdisciplinary team members (PT, OT, ST, MSW, etc.)
Perform treatments and procedures in compliance with nursing standards and regulations
Assess home safety and report concerns, including potential neglect or abuse
Complete accurate, timely documentation (OASIS/Medicare/Medicaid guidelines)
Participate in on-call rotation, weekends, and holidays as needed
Support team collaboration, quality improvement, and patient satisfaction initiatives
Qualifications
Graduate of an accredited nursing program
Current RN licensure
Valid driver's license
CPR certification
Preferred:
1+ year of relevant nursing experience (home health or related field)
