Summary

DEPARTMENT: Emergency Department         

REPORTS TO: CEO

General Summary: 

  • Provides intensive, goal directed services to support individuals who have a mental health and/or substance use crisis that results in the need for stabilization. The team works closely with all Pratt Regional Medical Center (PRMC) clinical and community partners to provide services at a higher level of intensity or frequency with the goal of community tenure by the consumers. 
  • Actively participates in outstanding customer service and accepts responsibility in maintaining relationships that are equally respectful to all.
  • Liaison between community partners including by not limited to PRMC, Horizons, Pratt Police Department, and the Pratt County Sherriff's Office.
  • This position will require crisis management response in Pratt County.
  • Includes Substance Use Disorder (SUD) outreach services and services for those in SUD crises.

Essential Functions:

  • Triage crisis response calls.
  • Provides person-centered case management utilizing identified and accepted methods to remediate symptoms of mental illness and/or improve emotional and functional well-being
  • Provide crisis stabilization services to individuals experiencing a mental health crisis in the community or while they are in the Crisis Stabilization Unit (CSU).
  • Conducts trauma-informed risk assessments as needed to address safety concerns.
  • Collaborates with community partners, caregivers, family members, physicians and other treatment providers regularly until the crisis episode is complete.
  • Support consumer achieve treatment goals, coordinate with collateral contacts, medication education, crisis management, prompt client to complete daily living activities, monitor client one to one for safety or as needed.
  • Participates in care coordination services with providers to request and arrange services; coordinate and monitor services rendered.
  • Create and initiate plans to ensure clients attend mental health appointments.
  • Work towards successful reintegration of consumers back into their home, community, school and outpatient/community mental health services.
  • Schedule may include working three (3) to five (5) days per week with possible on-call time.  Hours and days will be flexible as this is a new position.
  • Complete all required documentation in a timely and accurate manner as defined by PRMC.
  • Other activities as assigned.

Minimum Knowledge and Skills Required: 

  • Familiarity with Community Mental Health services
  • Ability to maintain confidentiality and adhere to professional boundaries.

Preferred Knowledge and Skills Required:

  • Two years' experience working with adults or children in a health or education related field.

Minimum Education and Experience Required: 

  • Bachelor's Degree or equivalent years of work plus experience in a related field
  • Must be 21 years of age.

Preferred Education and Experience: 

  • Bachelor's degree in Social Work, Sociology, Psychology, or Special Education (with specialized training in human development and family dynamics).
  • Three to five years' professional experience in mental health services, crisis response, or a related field.
  • Two years or more of experience working with educating and teaching clients about self-care, improving daily living activities.

Required License/Registration/Certification:

  • Employee must complete required on-line training, pass KBI, DCF child abuse check, adult abuse registry, and motor vehicle screens.   Required training in Adult Case Management.
  • Valid driver's license, insurance, and a dependable vehicle
  • Must be able to respond within 30 minutes to crisis call, during regular office hours and when on-call.
