Job Summary
Performs biopsycho-social assessments, creates and updates individualized treatment plans, provides individual, family and group therapy at various outpatient medical practices. Specializes in work with adults, and/or adolescents, and families.Accountabilities
- Completes an accurate biopsychosocial assessment, including competency in diagnostic determinations and determination of appropriate level of care for patient. – 5%
- Participates in regularly scheduled supervision with clinical supervisor – 5%
- Develops and individualized treatment plan including measurable treatment goals related to patient's presenting problems. Participates as an active member of the multi-disciplinary treatment plan. – 5%
- Provides the most appropriate therapy, specific to patient's symptoms. – 70%
- Performs any case management responsibilities involved with direct patient care. This includes, but is not limited to, Utilization Review, Discharge Planning, Family Involvement/Education, timely and appropriate documentation of care provided, communication with referral sources, as well as coordination of care with multidisciplinary treatment team and outside community providers. –15%
Supervisory/Management Responsibility
This is a non-management job that will report to a supervisor, manager, director, or executive.
Minimum Requirements
- Master's Degree - Counseling, or Marriage and Family Therapy
- Passed the National Counselor's Exam (NCE) or the National Clinical Mental Health Counseling Examination (NCMHCE)
Required Certifications/Registrations/Licenses
Must have submitted all required documentation to the Center for Credentialing & Education (CCE) for the Licensed Professional Counselor Associate designation by the time of onboarding.
Other Required Skills and Experience
- Group, individual, and family therapy skills, psychiatric problem-solving abilities, assessment skills, treatment planning skills. - Required
- Oral and written communication skills; interpersonal skills across multiple professions/disciplines. – Required
- Basic knowledge of computer software and utilization of electronic medical record. - Required
