Our community is looking for a Scheduling Coordinator to join our team.  

Responsibilities:  

  • Is responsible for the coordination of patient visits, maintenance and upkeep of scheduling records and log books.
  • Staffing coordination duties include the accurate and timely communication of scheduling changes between office and field staff.
  • Maintains a current client roster with necessary information.
  • Updates logs and schedule sheets.
  • Prepares weekly schedule for field staff and provides copies for distribution via email.
  • Assists in coordinating services provided to patients.
  • Controls and monitors schedule changes.
  • Obtains weekly visit count and reports results to the Clinical Supervisor.
  • Assists in relaying messages to field staff, office staff and community liaisons, gives and takes reports from the on-call coordinator.
  • Takes on call on rotation and as needed
  • Assist with confirming shifts and payroll as needed for private duty.
  • Ensure geographic scheduling to reduce drive time and to enhance staff satisfaction and patient care.
  • Accurately process new patient referral information into the computer on a timely basis and create/maintain patient calendar.
  • Acts as liaison among staff, patients, family, physicians, referral sources and payers
  • Maintains an ongoing relationship with current, future and former clients, client families, company sales team, employees and outside agencies.
  • Participates in company quality improvement activities.
  • Understands and adheres to established policies and procedures of agency, as well as compliance to local state and federal laws.
  • Answers telephone lines promptly, efficiently and channel them appropriately
  • Performs other duties as assigned by the Supervisor, DCS or Administrator.

Qualifications:

  • Home health experience required
  • Active Certified Medication Technician
  • High school diploma or equivalent required.
  • Demonstrates good communication and public relations skills.
  • Passionate for delivering a world class customer experience.
  • Internet service with minimum Broad band.
  • Must be able to travel if indicated for training and/or Discovery Communities/Properties.
