Overview
The Department for Children and Families (DCF) is recruiting for five limited-service Housing Case Manager positions to support Vermonters experiencing homelessness and help build DCF's new housing case management program within the Vermont Homelessness Response Continuum (VHRC).
Positions are available in Burlington (2), Rutland (1), Brattleboro (1), and Bennington (1).
Housing Case Managers will maintain a caseload and work directly with participants to assess needs, develop individualized housing plans, coordinate services, and support their progress toward stable housing.
This role offers a unique combination of office and field work, with case managers being based out of their local district office but spending the majority of their time traveling throughout their assigned region to meet with participants in hotels and community settings. They will work independently while receiving regular supervision and collaborating with DCF housing leadership, AHS Field Services, and community partners.
Responsibilities include determining eligibility, conducting regular participant visits, completing required documentation, and housing plans, coordinating with housing, medical, behavioral health, and community providers, and connecting participants with services and supports.
We're looking for candidates with strong communication, critical-thinking, interpersonal, and problem-solving skills who enjoy building relationships, working collaboratively, and helping individuals move toward stable housing.
This position, DCF Housing Case Manager - Limited Service (Job Requisition #55876), is open to all State employees and external applicants. It is a Limited Service position, which is non-tenured and authorized for a specific period of time. Limited Service positions are established for specially funded projects or programs.

AHS BACKGROUND CHECKS: Candidates must pass any level of background investigation applicable to the position. Environmental FactorsWork is performed in an office setting and in a variety of field situations. Private means of transportation should be available since travel to field sites will be required.
Minimum QualificationsMaster's degree in education, human services, health, counseling or related field.
OR
Bachelor's degree in education, human services, health, counseling or related field and one (1) year of experience or training in housing, human services, mental health, substance use, public health, education, counseling or related fields.
Preferred QualificationsExperience with community-based health/behavioral health and/or homeless assistance/prevention programs
