Position Summary

The Health and Wellness Director is responsible for overseeing the overall health and wellness of residents within the community. This includes clinical leadership, compliance with healthcare regulations, and management of nursing and care staff.

Key Responsibilities
  • Ensure compliance with all federal, state, and local regulations related to clinical care.

  • Monitor and report health incident trends; develop and implement improvement plans as needed.

  • Partner with the Executive Director and department heads to maintain resident and staff safety.

  • Collaborate with pharmacy consultants to ensure optimal medication management.

  • Provide clinical oversight and training to licensed nurses and care staff.

  • Complete resident assessments and update service plans as needed based on health status changes.

  • Communicate effectively with residents' families or responsible parties regarding health updates.

  • Maintain accurate documentation and ensure service plans comply with regulatory standards.

  • Oversee monthly staff scheduling and ensure adequate clinical coverage.

  • Ensure availability of necessary clinical supplies and participate in on-call rotations.

  • Support community outreach and engagement initiatives as a clinical leader.

  • Collaborate with Business Office for adjustments in care-related billing as needed.

Supervisory Responsibilities
  • Directly supervises Licensed Practical Nurses (LPNs), Certified Nursing Assistants (CNAs), and other care staff.

  • Ensures high standards of care and performance in accordance with company policies and applicable laws.

Qualifications

Education & Licensure:

  • Registered Nurse (RN) or Licensed Practical Nurse (LPN), current and in good standing (RN preferred).

  • CPR/BLS Certification required.

Experience:

  • Minimum 1 year of leadership experience in a healthcare or senior living setting.

  • Prior experience in assisted living or memory care preferred.

Skills & Competencies:

  • Strong clinical knowledge and judgment.

  • Excellent written and verbal communication skills.

  • Ability to use Microsoft Office and electronic health record systems.

  • High emotional intelligence and interest in working with older adults.

  • Proficient in interpreting medical documents, procedures, and medication guidelines.

Work Environment & Physical Requirements
  • Primarily indoor, climate-controlled work environment.

  • May involve exposure to infectious diseases, bodily fluids, unpleasant odors, and chemicals.

  • Must be able to lift up to 50 lbs independently and 100 lbs with assistance.

  • Ability to push residents in wheelchairs (up to 350 lbs).

  • Frequent standing, walking, reaching, and speaking; occasional kneeling, crouching, and stooping.
