Under the direction of the Health Information Management Supervisor and the Health Information Management Director. Responsible for entry of medical/psychiatric dictation by physicians regarding patient assessment, clinical course, and diagnosis, to document patient care and facilitate delivery of healthcare services. Employee actively participates and/or serves in a supporting role to meet the agency's obligations for disaster response and/or recovery or Continuity of Operations (COOP) activation.

Performs other duties as assigned. Other duties as assigned include but are not limited to actively participating and/or serving in a supporting role to meet the agency's obligations for disaster response and/or recovery or Continuity of Operations (COOP) activation. Such participation may require an alternate shift pattern assignment and/or location.

 

Hours Shift: 2:30pm-11:00pm

 

Essential Job Functions (EJFs):

 

Attends work on a regular basis and may be asked to work a specific shift schedule or, at times, even a rotating schedule, extended shift and/or overtime in accordance with agency leave policy and performs other duties as assigned.

 

Analyzes requests for medical information, copies, and sends them out in accordance with the facility and departmental policies and procedures. Assists patients, staff and affiliates with records requests for continuity of care purposes as well as other record-related questions

 

Gathers and sorts medical information and documents received from paper sources and systems and prepares for scanning into the electronic health record.

 

Creates digital images of paper records/documents and completes quality review prior to storing into the electronic record.

 

Other duties as assigned include but are not limited to actively participating and/or serving in a supporting role to meet the agency's obligations for disaster response and/or recovery or Continuity of Operations (COOP) activation. Such participation may require an alternate shift pattern assignment and/or location.”

 

Knowledge, Skills and Abilities (KSAs):

 

Skills:

Must have medical office/medical record department experience.

Excellent verbal and written communication skills.

Excellent organization and time management skills.

Must be detail oriented and ensure the quality and accuracy of patient records by following state hospital and department policies.

Must be able to use computer programs such as Microsoft Outlook, Word, and Excel.

 

Knowledge:

Knowledge in the review and spelling of medical/psychiatric terminology

Knowledge with federal and state guidelines governing the use and disclosure of protected health information.

Knowledge in the use of electronic medical records.

Proficient in operating office equipment, including high-volume scanners, photocopiers, and printers.

 

Abilities:

Must be able to work independently and productively with staff and external contacts.

Must be able to collaborate across departments to achieve timeframe completion objectives and goals.

Must have the ability to exchange ideas and engage in collective problem-solving.

Must be able to provide excellent customer service.

Must be willing to cross-train in other areas of the HIM Department.

Must be able to ensure that no information, facts, or comments learned in the performance of duty are discussed to protect the confidentiality of the patients served and/or Rio Grande State Center.

Must be able to type at least 40 words per minute.

 

Registrations, Licensure Requirements or Certifications:

 

If transporting patients, must possess a valid Texas driver license or obtain it no later than 90 days after hire date to drive a state vehicle. Applicants with an out-of-state driver license must provide an original certified driving record from the state of driving licensure. Eligible driving record required based on HHSC Fleet Management policy.

 

Initial Screening Criteria:

 

At least one (1) year experience in clerical work.

 

Must have medical office/medical record department job related experience 

 

Must be experienced in the use of computer programs such as Outlook, Word, and Excel. 

 

Must be able to type a minimum of 40 wpm

 

Additional Information:

 

Applicants must pass a fingerprint criminal background check, pre-employment drug screen, and registry checks including the Client Abuse/Neglect Reporting System (CANRS), Nurse Aid, Medication Aide and Employee Misconduct, List of Excluded Individuals/Entities (LEIE). Males between the ages of 18-25 must be registered with the Selective Service.

 

All applicants must be at least 18 years of age to be considered for employment at a state-operated facility. Waco Center for Youth applicants must be 21 years of age.

 

Flexibility in work hours may be required for this position. The position may be required to work overtime and/or extended hours.

Compliance with HHSC immunization policy and state hospital operating procedures related to immunizations is required. According to the Centers for Disease Control and Prevention, healthcare workers are considered to be at significant risk for acquiring or transmitting hepatitis B, measles, mumps, rubella, varicella (chicken pox), tetanus, diphtheria, pertussis (whooping cough), and influenza. All these diseases are vaccine preventable. As a result, state hospital policy requires employees be vaccinated according to their level of contact with individuals. In the event you choose to not be immunized for the influenza virus, you may be required to wear a mask and take other protective measures.
