The state supported living centers serve people with intellectual and developmental disabilities who are medically fragile or who have behavioral problems. The QIDP Coordinator works under the direct supervision of the Assistant Director of Programs (ADOP) or equivalent and has the primary responsibility for providing leadership and directions to QIDPs, in collaboration with their direct supervisors, to assure the delivery of quality program services that promotes dignity and respect for the individuals served.  The QIDP Coordinator is responsible for working cooperatively with other divisions and departments and for ensuring harmonious working relationships within his/her unit among staff, family members, and other facility employees. The QIDP Coordinator is responsible for planning and coordinating the use of all departmental resources.   The QIDP Coordinator is responsible for training and coordination of QIDP services and data processing services.  This position, at times may be required to manage a partial QIDP caseload.  The QIDP Coordinator performs complex consultative serves and provides technical assistance to Interdisciplinary Team (IDT) members.  Some flex time, overtime, evening and weekend work will be required.

 

Essential Job Functions (EJFs):

 

Attends work on a regular basis and may be required to work a specific shift schedule or, at times, even a rotating schedule, extended shift and/or overtime in accordance with agency leave policy and performs other duties as assigned. May serve in an on-call status on a rotating schedule as required to meet operational needs of the facility.

 

Provides leadership and support to all center QIDPs to ensure the delivery of quality services that promotes dignity and respect for SSLC individuals. Provides oversight and monitoring of all QIDP related duties ensuring that professional practices are in compliance with ICF/IID Regulations and the conditions of the DOJ Settlement Agreement. Ensures that QIDP services are delivered in a consistent, standardized manner across the center. This includes routine monitoring and review of Individual Support Plan (ISP) meetings and the related written documents in the electronic health record to provide feedback and support to the QIDPs to improve the quality of their work.

        

Provides leadership and support to ensure a knowledgeable and competent QIDP department. In conjunction with the Unit Director, the QIDP Coordinator participates in the screening, interviewing, and selection of all QIDP staff. Monitors the work performance of QIDP staff, providing feedback and input to the Unit Director for the performance evaluation of all QIDPs, including disciplinary action when appropriate. Develops and implements strategies to motivate, encourage, and equip QIDP staff. Promotes harmonious working relationships among QIDPs, Interdisciplinary Teams, family members, and other center employees.

        

Develops, revises, and maintains policies/procedures relating to QIDP duties and the ISP development, implementation, and monitoring process. Ensures QIDPs are knowledgeable of and effectively implementing these practices.

 

Educates, encourages, and supports all QIDPs through the coordination and oversight of departmental training efforts. This includes coordination of orientation and training for newly hired QIDPs and ongoing training for all QIDP staff on their role. Serves as the SSLCs ISP discipline coordinator and provides training, support, and guidance, as appropriate and when requested, to all staff regarding the ISP process and any revisions or process changes. 

 

Performs complex training, monitoring, tracking, trending, and analysis of data regarding outcomes for SSLC individuals and QIDP/IDT activities. Works together in coordination with other departments to develop and implement processes and practices to ensure positive outcomes based upon data trends and analysis. This includes the development of plans of correction, timely submission of evidence, and participation in the Quality Assurance/Quality Improvement Council.

 

Participates in various committees and work groups designed to ensure SSLC individuals are receiving quality services and care. Works with other departments to ensure service coordination and consistent development of holistic individual plans. Takes a proactive approach to identifying problems that affect the provision of services. Coordinates efforts to develop solutions and follows-up until problems are resolved. Performs after-hours monitoring and other monitoring duties as assigned to ensure consistent program coordination and implementation. Performs special projects and other duties as assigned. As needed, to ensure continuity, this position may be required to manage a QIDP caseload. 

 

The QIDP Coordinator is expected to participate in mandatory training per HHSC policies and procedures, HIPAA policies, state and federal rules and regulations, ICF-IID regulations, and the requirements in the SSLC's Department of Justice Settlement Agreement in order to provide quality services to individuals served.

 

Other duties as assigned include but are not limited to actively participating and/or serving in a supporting role to meet the agency's obligations for disaster response and/or recovery or Continuity of Operations (COOP) activation.  Such participation may require an alternate shift pattern assignment and/or location.

 

Knowledge, Skills and Abilities (KSAs):

 

Knowledge of ICF/IID standards, SSLC policy, and the Settlement Agreement with the U.S. Department of Justice (DOJ).

 

Knowledge of the Interdisciplinary Team (IDT) process.

 

Knowledge of people-centered planning.

 

Knowledge of rehabilitation and skills acquisition programs for persons with intellectual disabilities, including program design and collecting and analyzing data.

 

Knowledge of discipline specific plans and processes such as Positive Behavior Support Plans (PBSP) and Physical Nutritional Management Plans (PNMP).

 

Knowledge of the role and responsibilities of the QIDP at an Intermediate Care Facility for Individuals with Intellectual Disabilities (ICF-IID).

 

Knowledge of leadership and management tools.

 

Knowledge of education/training and active treatment methods, tools, techniques, and processes.

 

Ability to coordinate programmatic and clinical services and supports.

 

Ability to prepare and maintain records.

 

Ability to quickly adapt to change.

 

Ability to communicate verbally and in writing in a clear, effective, concise and constructive manner.

 

Ability to assess and solve problems.

Initial Screening Criteria:

 

Bachelor's degree from an accredited college or university with a major in psychology, sociology, social work, special education, rehabilitation counseling, or a related human services field such as therapeutic studies (e.g., music, art, occupational, physical therapy), recreation, or a combination of equivalent subject matter. For a related human services field, the transcript must be reviewed for applicable coursework that meets federal guidelines for QIDP designation.

 

At least three (3) years of experience working directly with persons with intellectual disabilities or other developmental disabilities.

 

At least one (1) of experience as a QIDP is preferred.

 

Experience training, leading the work of others or supervising others is preferred.

Flexibility in work hours may be required for this position. The position may be required to work overtime and/or extended hours. Must be willing to work weekends and holidays.
