Provides education services to nursing staff in preparing for contemporary practice and supporting them in acquiring the knowledge and skills so they can competently carry out all facets of patient education related to wound/ostomy care and diabetes. Utilizes theory-based practice to include nursing care to hospitalized patients with complex wounds and ostomies, performs assessments and develops treatment plans to optimize wound healing. You are part of an interdisciplinary team providing education, support and continuity of care throughout the various stages of treatment and follow-up.

  • Current New York State RN License/Registration
  • BLS Certification
  • Certified Wound-Ostomy Nurse (CWON) required
  • Certified Diabetes Educator (CDE) preferred


  • Bachelor's in Nursing (BSN) required
  • Master's Degree in Nursing or education preferred
  • Maintains a minimum of 30 hours of continuing education.


  • A minimum of 3 years of Clinical RN experience
  • Extensive Wound/Ostomy care experience
  • Strong medical-surgical background preferred


  • Implement the nursing process for medical and surgical patients by performing comprehensive assessments, developing individualized plans of care and carrying out targeted wound care interventions to foster healing
  • Collaborate with a multi-disciplinary team to facilitate continuity of care.
  • Provide exceptional patient centered care to our patients, which include wound, ostomy, and diabetic care.
  • Ensure that all wounds and ostomies are appropriately managed through proper assessments and treatments.
  • Ensure weekly wound rounds are conducted with a clinical team and all necessary reports, and documentation are completed
  • Provide in-services on wound, ostomies and diabetic management
  • Responsible for orientation and skills lab training for diabetes management and education
  • Provides direct patient education and coordination of care in planning for discharge and transition to outpatient services.
  • Evaluates and develops patient educational materials and gives patient and/or family members instructions on diabetes self-management education.
  • Trains, oversees and provides current patient information to nurses and other health care personnel in order to assure comprehensive patient care.
  • Participates in achieving length of stay goals for diabetes population by reviewing hospital/unit glycemia results.
  • Demonstrates advanced skills in assessment of educational needs and interventions for patients, cognizant of psychosocial and cultural implications.
  • Promotes evidence-based practice findings and identifies clinical research opportunities in collaboration with staff nurses.
  • Implements teaching and learning strategies and interventions based on adult learning theory, developing teaching skills among nurses.
  • Evaluates clinical outcomes as related to education interventions including the impact on cost, quality and satisfaction with health care.
  • Demonstrates a high level of collaborative practice and excellent communication skills.
  • Functions autonomously and is an independent decision maker, able to practice within an acute care setting while demonstrating outstanding skills in working with the acutely ill elderly and chronically ill clients.
  • Displays a professional demeanor that exemplifies a positive role model for professional nursing practice at all times.
  • Functions as a facilitator and innovator for state of the art patient education with direct involvement in the implementation of new techniques for quality care.
  • Maintains own knowledge base to keep current on interventions, trends, healthcare and professional nursing development.
  • Actively guards the confidentiality of sensitive information including but not limited to the patients, staff and the health system.


  • Reports directly to the Director of Professional Practice, Education, and Research.
  • Collaborates with the clinical nurse educator in creation of clinical skills and nursing professional development.




  • Collaborates with all heath care disciplines in evaluating the impact of education and various clinical interventions to enhance clinical outcomes.
  • Ensures that educational activities are congruent with Thompson Health's mission, goals, values, priorities and resources.



  • Collaborates closely with nurse directors, physicians, staff members and all interdisciplinary team members to ensure over-all quality patient care.
