- This position is based in Sidney and may require occasional travel to healthcare facilities and patient homes, within the following 6 county radius of Auglaize, Champaign, Darke, Logan, Miami and Shelby Counties.
- The RN Case Manager will work in a fast-paced environment that demands flexibility, critical thinking, and a patient-centered approach.
- Use the nursing process and perform initial holistic nursing assessment of the client.
- Establish short and long term goals, provide skilled nursing service in the home.
- Document Oasis accurately and completely.
- Complete Necessary forms required for payer billing.
- Assess safety of the patient environment.
- Develop a comprehensive plan of care based on nursing assessment and oasis.
- Complete all referral forms to appropriate referrals.
- Review, teach, and administer medications in a safe and timely manner.
- Recognize and utilize opportunities for health teaching and provide patient education.
- Current and valid Registered Nurse (RN) license.
- Minimum of 2 years nursing experience, preferably in case management or related field.
- Strong knowledge of healthcare systems, insurance processes, and patient advocacy.
- Excellent communication, organizational, and problem-solving skills.
- Ability to work independently and as part of a multidisciplinary team.
- Proficiency with electronic medical records (EMR) and case management software is a plus.
- Previous Home Health/Hospice experience preferred, Medical/Surgical and Long Term Care experience helpful
