Practice Details:

Allina Health Home and Community Services is seeking b/c or b/e Geriatrician, Family Medicine or Internal Medicine Physician to join the Senior Health Co-Care Team.

The complex care team is a robust multi-disciplinary team which includes:

• Physician
• Full-time Nurse Practitioner
• Nurse triage 24/7
• Advanced Care Team: RN care coordinator, social worker, pharmacist, and care guide.

About the practice:

• You will be part of a valued program within Allina Health that provides easy access for seniors to receive primary care in their home.
• In this role you will see patients in the North Metro area of the Twin cities with a patient panel of 100 patients.
• Excellian, our EMR, is a software product from Epic and is considered one of the most comprehensive systems in the nation.

Qualifications:

• Active, unrestricted State of MN Medical License
• NPI and DEA required
• Must be BE/BC
