Overview

Sanger Heart and Vascular Institute's Advanced Heart Failure program is seeking a full-time advanced practice provider (APP) to join our growing team at Atrium Health. This position is occupied by board certified nurse practitioners and/or physician assistants specifically trained and proficient in providing cardiac services under the direct supervision of Heart Failure/Transplant Cardiologists. The provider in this position will function at the top of their license in a hybrid inpatient and outpatient clinic-based setting in collaboration with a supervising physician and other members of a multidisciplinary team. The position will be split approximately 75% outpatient and 25% inpatient. The outpatient component will include helping to build and support a heart failure guideline directed medical therapy titration clinic providing both face-to-face and virtual services to heart failure patients with the goal of rapid medication titration and evaluation for device or advanced heart failure therapies if indicated. Other outpatient coverage locations may include longitudinal heart failure and heart success clinics as needed providing care to patients who have advanced systolic heart failure and/or diastolic heart failure from ischemic, nonischemic, infiltrative, structural, congenital or various other cardiomyopathies. The inpatient component will occur on a rotational basis and include admissions, daily rounds on both floor and ICU patients, and discharging patients from the acute care setting.

Position Description:

  • APP will work closely with a multidisciplinary team to provide comprehensive patient care in various outpatient clinical settings and inpatient on a rotational basis
  • Responsible for helping to build and support a heart failure guideline directed medical therapy titration clinic
  • Evaluate new patients, consults and established patients as well as diagnose, treat and manage common conditions in the cardiovascular population.
  • Willingness to provide coverage at SHVI Kenilworth, SHVI Pineville, and SHVI Cabarrus as needed
  • Perform an appropriate history and physical exam, assess, and prepare patient workups and follow core measures for documentation including hospital and 3rd party payer requirements and appropriate patient care.
  • Facilitate and order diagnostic studies including echocardiograms, stress tests, MRIs, CTs, and cardiac catheterization if indicated.
  • Order appropriate laboratory studies and diagnostic procedures. Explained necessity, preparation, and risks and benefits of scheduled diagnostics and therapeutic procedures to the patient and family.
  • The ability to recognize emergency situations and respond appropriately.
  • Participate in evaluations with the primary supervising physician as directed by State laws.
  • Maintain BLS (Basic Life Support) and ACLS (Advanced Cardiac Life Support) certification as well as all licenses and certifications as mandated by the Hospital Credentialing Committee.
  • Exhibits respect for others by displaying a positive, courteous attitude at all times.
  • Participates in staff meetings, in-services, and continuing education for job related growth.
  • Shifts: 1st shift M-F. S/Su coverage from 7am-12pm on a rotational basis. No call.
  • Eligible Performance Incentive Bonus annually. $2500 for and 40h for CME.

  • Medication therapy and management

Position Qualifications:

  • Graduation for an accredited NP program. Must be approved to practice as a Nurse Practitioner in the state of North and South Carolina. Current prescriptive privileges and DEA license required. BLS and ACLS for Healthcare Provider from AHA required with expiration date greater than 90 days from date of beginning practice.
  • Nurse Practitioner: Master's Degree or Doctorate of Nursing Practice required. License to practice as a Registered Nurse required. Certification in practice from a nationally recognized certifying body required. ACNP, AGACNP certification preferred based on population served. ** Must sit for and pass Certified Heart Failure Nurse (CHFN) examination within 2 years of beginning practice
  • Cardiology experience required; prior heart failure experience preferred
  • Demonstrate appropriate critical thinking and interpersonal skills when working in a faced paced, high acuity work environment. Able to perform a thorough history and physical assessment.
