Description
Overview
Provides psychosocial assessments, support, short-term counseling and social work services to cancer patients and their loved ones throughout the continuum of care including diagnosis, treatment, survivorship, palliative care, end-of-life care, and bereavement. Serves as the subject matter expert on oncology psychosocial distress screening and leads the development and maintenance of a screening program; works with financial counselors to meet the financial, transportation, and medication needs of patients. Responsible for program development and maintenance for support groups and survivorship activities, including sharing in the responsibility for facilitation of these groups. Works collaboratively with other Oncology Care Team members to facilitate care of the patient and family across the many disciplines and professionals required for their care.

Experience
2+ Oncology Social Work Required; 2+ Counseling, Support Group Facilitation Preferred
