Summary of Essential Functions for the Position:
The Post-Acute Care Coordinator I (PACC) is responsible for the facilitation of a seamless transition for patients
discharging from a facility setting to one that facilitates post-acute needs, by following, communicating and collaborating
with both internal and external teams to support patient-centric care. The PACC I will assess patients to determine their
level of health literacy, assess for patient and caregiver needs, coordinate between the referral sources and discharge
provider to assure needed documentation is obtained, and ensure patients and families are included in care planning. Once
the transition is accomplished, the PACC I will follow and coordinate with the Agency to ensure that there are patient
centric plans in place for optimal patient outcomes. The PACC I must possess the ability to communicate and collaborate
with individuals in many different settings, utilizing not only clinical, but also customer service, problem solving and
analytical skills that lead to market development initiatives and growth while focusing on patient satisfaction.
Qualifications / Licensure / Certification / Knowledge / Skills
A minimum of one (1) year experience in a health care organization, home care preferred
Currently licensed in the state(s) in which practicing as a Licensed Practical Nurse (LPN), Physical Therapy
Assistant (PTA) or Occupational Therapy Assistant (OTA) required
CPR certified with American Heart Association or American Red Cross
May be required to assist the assigned branch with direct-patient care, from time-to-time, based on the needs
of the branch and at the Executive Director of Care Coordinations discretion.
Possess excellent written, verbal and listening communication skills
Ability to manage conflict, stress and multiple simultaneous work demands in an effective, professional
Ability to work independently, while collaborating with other team members, build relationships and be
Strong understanding of customer and market dynamics, as well as transitional care best practices
Possess sound organizational skills to include time management and problem solving
Experience with State, Federal and CMS guidelines and regulations CMS
Must demonstrate good customer relations skills and a commitment to providing quality service
Familiarity with healthcare laws, regulations, multiple accreditation standards and elements of performance
Proficient with a computer and Microsoft Word, Excel and Outlook software. Working knowledge of EMR
Ability to work with culturally diverse clients and address low literacy issue in care provision
Acceptance and ability to demonstrate and support the core values and goals of Agency
Valid drivers license, automobile liability insurance and reliable transportation required
Travel between facilities, hospitals and home care agency is required. Approximately 75%-100% travel.
High School Diploma or GED. LPN - Graduate of a state approved school of practical (vocational) nursing.
Physical Therapist Assistant or Certified Occupational Therapist, an Associates degree in Allied Health -
Physical or Occupational Therapy.
Working Conditions & Physical Requirements:
Work environment is relatively quiet and can be stressful due to deadlines, multiple tasks and general
compliance of law, rules and regulations. The position requires visual acuity and dexterity, sitting, standing,
some pushing, pulling and lifting up to 25 pounds. Auto related accidents possible.
Direct-Patient Care Working Conditions & Physical Requirements:
Works indoors in Agency office and patient homes and travels to/from patient homes. Requires considerable
physical effort most of the day including kneeling, squatting, reaching, twisting, climbing, walking, exposure to
temperature and humidity changes and maximal assist in lifting and/or transferring of a 100-pound patient. Must
possess sight/hearing senses or use appropriate adaptive devices that will enable senses to function at a level
required to meet the essential duties of the position. Must provide evidence of annual TB test and other staterequired tests or examinations. Be able to tolerate exposure to elements including, but not limited to, odors,
blood, body fluids and excrements, adverse environmental conditions and hazardous materials
